临床应用甲氨喋呤治疗异位妊娠两种方法的临床比较

摘　　要：目的探讨临床应用甲氨喋呤治疗异位妊娠快捷、有效的用药方法。方法随机选取2015年1月—2016年6月该院收治的100例临床确诊的初次异位妊娠患者,随机分为两组,A组给予甲氨喋呤连续5 d,1次/d肌内注射,B组给予较高浓度一次性肌注甲氨喋呤冲击治疗,观察并比较两组患者的病情变化以及出现的药物不良反应情况。结果两组患者在治疗两周后,依据A、B两组患者血β-HCG检测水平降低情况及超声检查孕囊变化为治愈指标,A组治愈率为76.7%,B组治愈率为93.3%,B组治愈率明显高于A组,且两组间比较差异有统计学意义(P<0.05)。A、B两组患者药物不良反应发生率分别为6.7%和10.0%,差异无统计学意义(P>0.05)。结论临床应用甲氨喋呤治疗异位妊娠,较高浓度一次性肌注冲击疗法较肌内注射持续给药方法更快捷,更有效,值得临床推广。Objective To study th e rapid an d effective m ed ic at io n method of m e tho p ter in in tre a tm en t of ec to p ic g e s ta t io n . Methods 100 c a se s of p a t ien ts with in i t ia l ec to pic ges tat io n adm it ted and tre a ted in our h ospi tal from Jan u a ry 2015 to Ju n e 2016 were random se le c ted and randomly d iv id ed into two g ro up s, th e group A ad o pted th e in tram u sc u la r in je c t io n of methopter in for 5 co n se cu tiv e days once a d ay, th e group B ad o pted th e o n e - t im e high co n c en tra t io n in tram u sc u la r p ulse treatment of me th o p te r in, and th e ch ang es of d ise a se and ad v erse reac tio n s of drugs of th e two groups were o bserved and compared. Results After 2-w e e k tre a tm en t, th e cu re ra te in th e group B was obviously h ig he r th an th a t in th e group A（6.7% vs 10.0%）, an d th e d ifferenc e h ad s ta t is t ic a l sign if ic an ce （ P 〈0.05）, an d th e in c id e n c e ra te of ad v erse re a c tio n s of drugs in the group A an d group B was re sp e c tiv e ly 6.7% and 10.0%, b u t th e differenc e h ad no s ta t is t ic a l s ignif icance （ P 〉0.05）. Conclusion On e - t ime high co n c en tra t io n in tram u sc u la r p ulse tre a tm en t of m eth o p te r in is more ra p id and effective th an the intramuscular in je c t io n co ntinuous adm in is tra t io n m etho d, which is worth c l in ic a l p romotion.
