摘 要:目的探讨低剂量双源CT(DSCT)双相肺容积及密度指标与肺功能的相关性,评估其在慢性阻塞性肺疾病(COPD)中的价值。方法方便选取2014年8月—2016年9月期间福州肺科医院CT室确诊COPD患者60例为COPD组,健康志愿者45名作对照组,均进行最大吸气末和最大呼气末的低剂量双相DSCT扫描,比较两组DSCT各项容积及密度指标的差异,分析其与肺功能指标的相关性。结果 COPD组最大吸气末容积Vin:(5 229.18±1 268.26)m L,最大呼气末容积Vex:(3 507.55±1 208.74)m L,双相肺容积差(Vin-Vex):(1 721.63±852.21)m L,最大吸气末平均肺密度MLDin:(-816.23±26.03)HU,最大呼气末平均肺密度MLDex:(-742.79±56.85)HU,双相平均肺密度差(MLDinMLDex):(-73.45±53.82)HU,与对照组比较,Vin增高、Vex增高更明显,(Vin-Vex)明显减少,MLDin、MLDex明显下降,(MLDin-MLDex)明显减少。相关性分析中,两组Vin、(Vin-Vex)均与FVC显著相关(P<0.01),Vin、MLDin与FEV1%预相关,与FEV1/FVC相关性更好(r=-0.593、0.444,P<0.01、<0.01),Vex与FEV1%预、FEV1/FVC显著相关(r=-0.451、-0.589,P<0.01、<0.01),MLDex与FEV1%预、FEV1/FVC显著相关(r=0.485、0.517,P<0.01、<0.01)。(MLDin-MLDex)与FEV1%预、FEV1/FVC相关(r=-0.382、-0.332,P<0.01、<0.01)。结论低剂量DSCT肺容积及肺密度与肺功能指标具有相关性,可以有效地评估慢性阻塞性肺疾病患者的肺功能。Objective To study the correlation between the low-dose dual-source CT lung volume, lung density index and lung function and evaluate the value of it in the chronic obstructive pulmonary disease. Methods 60 cases of COPD patients diagnosed in Fuzhou Pulmonary Hospital CT Room from August 2014 to September 2016 were conveniently selected and45 cases of healthy volunteers were selected as the control group, and the two groups underwent the low-dose biphasic DSCT scan of maximum end-inspiratory and maximum end-expiratory, and he differences in the DSCT various volumes and density indexes were compared between the two groups, and the correlation between it and lung function index was analyzed. Results The maximum end-inspiratory volume in the COPD group and maximum end-expiratory volume were respectively( 5 229.18±1 268.26)m L and(3 507.55±1 208.74)m L, and the dual-source lung volume difference(Vin-Vex):(1721.63±852.21)m L, and the mean lung density of maximum end-inspiratory MLDin:(-816.23±26.03)HU and the mean lung density of maximum end-expiratory MLDex:(-742.79±56.85)HU, and the mean biphasic lung density difference(MLDinMLDex):(-73.45±53.82)HU, and the Vin increased compared with that in the control group, and the Vex increased more obvious, and the(Vin-Vex) decreased more obviously, MLDin and MLDex decreased more obviously,(MLDin-MLDex) obviously decreased, and the correlation analysis showed that the Vin and(Vin-Vex)was obviously correlated with the FVC(P<0.01), and the Vin and MLDin had the pre-correlation with the FEV1% and the correlation with FEV1/FVC was better(r=-0.593,0.444, P<0.01,<0.01), and the Vex was obviously correlated with the FEV1% and FEV1/FVC(r=-0.451,-0.589,P<0.01,<0.01), and the MLDex was obviously correlated with the FEV1% and FEV1/FVC(r=0.485,0.517, P<0.01,<0.01), and the(MLDin-MLDex) was correlated with the FEV1% and FEV1/FVC(r=-0.382,-0.332, P<0.01,<0.01).
