运用Endobutton系统治疗TossyⅢ型肩锁关节脱位

摘　　要：目的针对TossyⅢ型肩锁关节脱位采用Endobutton系统治疗的临床效果分析。方法方便选取2011年10月—2015年10月期间该院创伤骨科收治100例TossyⅢ型肩锁关节脱位患者,平均分成两组,观察组50例,对照组50例,观察组进行Endobutton袢钢板治疗,对照组进行锁骨钩钢板治疗,对比两组患者手术时间、临床效果、伤口长度、住院时间、术后感染人数、术后肩关节活动程度等。结果观察组的伤口长度、肩关节疼痛人数、住院天数均低于对照组,且肩关节外展度优于对照组,差异有统计学意义(P<0.05),对照组出现1例术后内固定失效,但经再次住院锁骨钩钢板治疗,肩锁关节部分复位,观察组临床总有效率(93.33%),对照组临床总有效率(73.33%),观察组优于对照组,差异有统计学意义(P<0.05)。结论 Endobutton系统与锁骨钩钢板都可以治疗TossyⅢ型肩锁关节脱位,但在切口长度、术后肩关节恢复状态、肩关节疼痛率等方面,均优于锁骨钩钢板,而且Endobutton袢钢板治疗有利于肩关节早期功能锻炼,肩关节复位效果好,避免二次手术给患者带来痛苦,值得临床借鉴。Objective To analyze the clinical effect of Endobutton system in treatment of Tossy Ⅲ dislocation of acromioclavicular joint. Methods Convenient selection 10 cases of patients with TossyⅢ dislocation of acromioclavicular joint admitted and treated in our hospital from October 2011 to October 2015 were selected and divided into two groups with 50 cases in each, the observation group were treated with Endobutton stumbles steel plate, while the control group were treated with clavicular hook plate, and the operation time, clinical effect, cut length, length of stay, postoperative infection and postoperative shoulder joint degree of activity were compared between the two groups. Results The cut length, shoulder joint pains number and length of stay in the observation group were lower than those in the control group, and the shoulder joint abduction degree was better than that in the control group, and the difference was statistically significant(P<0.05), in the control group, the internal fixation of 1 case after operation was ineffective, but the shoulder joint part was reset after the second treatment, and the total clinical treatment effective in the observation group was better than that in the control group(93.33% vs 73.33%), and the difference was statistically significant(P<0.05). Conclusion Both Endobutton system and clavicular hook plate can treat the TossyⅢ dislocation of acromioclavicular joint, but the cut length, postoperative shoulder joint recovery state and shoulder joint pains are better than those of clavicular hook plate, and Endobutton stumbles steel plate treatment is conducive to the early function exercise of should joint with good shoulder joint reset effect and avoid the pains brought by the second operation, which is worth clinical reference.
