摘  要:目的探讨产妇术前静脉滴注不同浓度葡萄糖溶液对择期剖宫产新生儿低血糖的影响。方法选取2014年1月—2015年12月于上海交通大学附属第六人民医院行择期剖宫产手术的产妇300例,随机分为无糖组、高糖组和低糖组各100例,均于术前3 h开始静脉输液,持续至手术开始。无糖组输注不含葡萄糖的乳酸钠林格注射液500 m L,高糖组输注5%乳酸钠林格葡萄糖注射液500 m L,低糖组输注1%乳酸钠林格葡萄糖注射液500 m L。比较3组产妇术前10 min静脉血血糖值、胰岛素值;3组新生儿脐动脉血血糖值、胰岛素值,出生后30 min血糖值以及新生儿低血糖发生率。结果无糖组产妇术前10 min静脉血血糖值、新生儿脐动脉血血糖值低于高糖组和低糖组,差异有统计学意义(P<0.05),高糖组与低糖组比较,差异无统计学意义(P>0.05)。无糖组产妇术前10 min静脉血胰岛素值、新生儿脐动脉血胰岛素值低于低糖组,低糖组低于高糖组,差异均有统计学意义(P<0.05)。低糖组新生儿低血糖发生率最低(P<0.05)。无糖组新生儿产后30 min血糖值低于高糖组,高糖组低于低糖组,差异均有统计学意义(P<0.05)。结论择期剖宫产术前3 h静脉滴注1%乳酸钠林格葡萄糖注射液可有效降低新生儿低血糖发生率,值得临床推广。Objective To investigate the effect of intravenous infusion of glucose saline on neonatal hypoglycemia before elective cesarean delivery. Methods A total of 300 healthy pregnant women in Shanghai Sixth People' s Hospital, Shanghai Jiao Tong University were selected from January 2014 to December 2015 They were randomly and equally divided into three groups. Intravenous infusion was administered till 3 h before delivery. The group A received intravenous infusion of 500 mL sodium lactate Ringer' s injection without glucose, group B received intravenous infusion of 500 mL sodium lactate Ringer' s injection with 5 % glucose, and group C received intravenous infusion of 500 mL sodium lactate Ringer' s injection with 1% glucose. The maternal venous blood glucose and insulin levels 10 min before delivery; neonatal umbilical artery blood glucose and insulin levels at birth and blood glucose level 30 min after birth; and neonatal hypoglycemia rate among three groups were compared. Results The maternal venous blood glucose level 10 min before delivery and neonatal umbilical artery blood glucose level at birth in group A were significantly lower than those in groups B and C ( P 〈 0.05 ), and there was no significant difference between group B and group C. The maternal venous insulin level 10 min before delivery and neonatal umbilical artery blood insulin level at birth in group A were significantly lower than those in groups B and C ( P 〈 0. 05 ), and the values in group C were significantly lower signifi compared with group B (P 〈 0.05). The neonatal blood glucose level 30 min after birth in group A was cantly lower than that in groups B and C ( P 〈 0.05 ), and the value in group B was significantly lower compared with group C (P 〈 0.05 ). The incidence of neonatal hypoglycemia in group C was the lowest ( P 〈 0.05 ).
