认知疗法在创伤性截瘫合并抑郁患者中的应用

摘　　要：目的探讨认知疗法对创伤性截瘫合并抑郁患者中的应用效果。方法选取100例创伤性截瘫合并抑郁患者,按随机数字表法分为观察组和对照组,每组各50例。对照组给予常规的护理干预,观察组在常规护理基础上给予认知疗法干预。比较两组患者干预前后的焦虑、抑郁,心理卫生状况,自杀态度和生活质量。结果干预后观察患者焦虑、抑郁评分均低于对照组,观察组患者的心理卫生状况评分低于对照组,干预后观察组的自杀态度评分高于对照组和生活质量评分高于对照组,两组比较,差异具有统计学意义(均P<0.05)。结论通过认知疗法对创伤性截瘫合并抑郁患者进行干预,可显著降低患者自杀意念,改善患者焦虑及抑郁症状及心理卫生状况,提高生活质量,值得推广应用。Objective To explore the effect of cognitive therapy on patients with traumatic paraplegia. Methods Toally 100 traumatic paraplegia patients were randomly divided into observation group and control group with 5 0 cases in each. The control group received routine nursing intervention and the observation group cognitive therapy on the basis of intervention for the control group. The two groups were compared in terms of anxiety, depression, mental health, suicidal ideation and quality of life by the instruments of self-rating anxiety scale （SAS）, self-rating depression scale （SDS） and suicide attitude questionnaire （SQA） before and after intervention. Results Before the intervention, the scores by SDS, SAS scores and SQA were insignificant different between the groups （P〉0.05）. After the intervention, the scores on suicidal ideation in the observation group were significantly higher and the quality of life were significantly higher and the depresion and anxiety and the mental health status were lower than those of the control group （P 〈0.05）. Conclusion The cognitive therapy for the patients with traumatic paraplegia can significandy alleviate their depression and anxiety, reduce their suicidal ideation and improve the quality of life.
