Early Assessment of Coronary-artery Bypass Graft with the Perioperative Ultrasonography of the Internal Mammary Artery

摘　　要：目的对比分析冠状动脉旁路移植术病例的术中及术后乳内动脉(IMA)超声血流参数,通过对其在围手术期的动态变化的探讨分析,早期评估移植功能。方法冠状动脉旁路移植术患者75例,术中TTFM测量乳内动脉血流量、搏动指数、舒张分数。术后5~8d观察乳内动脉起始段,测量血流量、搏动指数、收缩期和舒张期峰值流速,计算舒张期与收缩期峰值流速比,对不同时期血流参数进行比较。结果 75例均于术中实施TTFM,术后5~8d乳内动脉起始段的显示率96%。72例术中TTFM与术后超声血流参数比较,乳内动脉血流量、搏动指数均具有显著差异。结论术中TTFM结合术后乳内动脉超声检查可早期评估冠状动脉旁路移植功能。Objective To make an early assessment of the graft patency through the analysis of intraoperative and postoperative flow parameters of internal mammary artery （IMA） in patients undergoing coronary artery bypass graft- ing with Ultrasonography. Methods 75 cases underwent coronary artery bypass grafting. The quantity of IMA flow, pulsatility index and diastolic filling percentage were measured with TTFM. The quantity of the initial segment of IMA, pulsatility index, systolic peak velocity, diastolic peak velocity, D/S were measured with Ultrasonography 5--8 days after coronary artery bypass grafting, the flow parameters of different stages were comparatively analyzed as well. Results TTFM was performed in all the 75 cases and the display rate of the initial segment of the internal mam- mary artery in 5--8 days was 96O/oo. There were significant differences in the quantity and pulsatility index of IMA in 72 cases with TTFM and postoperative Ultrasonography. Conclusions Intraoperative TTFM combining with postoperative Ultrasonography can be used to make an early assessment of coronary artery bypass graft.
