摘  要:目的探讨新诊断2型糖尿病患者不同体重指数(BMI)和年龄与胰岛β细胞功能及胰岛素抵抗的关系。方法内分泌科住院及门诊的新诊断2型糖尿病患者217例,依照不同的BMI水平分为正常组、超重组、肥胖组,分别比较各组血压、体重指数、腰围、血脂、血糖及静脉葡萄糖耐量试验(IVGTT)中的胰岛功能状态,并用稳态模型评估胰岛β细胞功能及胰岛素抵抗指数(HOMA-IR)。结果超重和肥胖组HOMA-IR及胰岛B细胞功能指数均明显高于正常组,不同年龄组HOMA-IR及胰岛β细胞功能指数无差异,肥胖组年龄超过60岁者急性胰岛素分泌指数及胰岛素曲线下面积高于年龄小于60岁者。结论新诊断2型糖尿病中非肥胖者胰岛素分泌功能缺陷更明显,肥胖者以胰岛素抵抗程度明显。未发现年龄与胰岛β细胞功能或胰岛素敏感性存在明确相关性。Objective To explored the relationship between age,body mass index (BMI) and islet B cell function,insulin resistance in the newly diagnosed type 2 diabetes patients. Methods 217 cases of newly diagnosed type 2 diabetes patients were divided into normal weight, overweight, and obese groups. The blood pressure, body mass index, waist circumference, blood lipids, blood glucose and islet function status reflected by intravenous glucose tolerance test (IVGTT) were compared among groups. Islet B cell function and insulin resistance index (HOMA-IR) were assessed using homeostasis model. Results HOMA- IR and pancreatic B cell function index in the overweight and the obesity group were significantly higher than the normal BMI group, HOMA-IR and pancreatic B cell function index showed no difference in different age groups of patients. In the obese group, the acute insulin secretion index and insulin area under the curve in the patients more than 60 years of age are higher than those younger than 60 years old. Conclusion In newly diagnosed type 2 diabetes patients, insulin secretion defect is more evident in non-obesity patients, insulin resistance is more evident in obesity patients. We do not find any clear relationships between the age and islet B cell function or insulin sensitivity.
