摘 要:目的探讨子痫前期孕妇孕中期血脂水平,分析早发型子痫前期的危险因素。方法检测62例早发型子痫前期孕妇(子痫前期组)和456名健康孕妇(对照组)孕24~28周血清甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白(apo)A1、apo B100、游离脂肪酸(FFA)、脂蛋白(a)[Lp(a)]水平,计算孕14~18周时的体重指数(BMI)。早发型子痫前期的危险因素分析采用多因素Logistic回归分析。结果与对照组比较,子痫前期组BMI、收缩压、舒张压、TG、FFA、Lp(a)、HDL-C、LDL-C、apo B100水平明显升高(P<0.05),其他指标2个组之间差异均无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,在校正年龄和孕周后,孕14~18周BMI[比值比(OR)=1.46,95%可信区间(CI)1.20~1.78)]、Lp(a)(OR=1.01,95%CI 1.00~1.02)、apo B100(OR=3.27,95%CI 1.07~10.67)、收缩压(OR=4.71,95%CI 2.05~9.32)、舒张压(OR=3.42,95%CI 1.34~8.64)是早发型子痫前期的危险因素。结论妊娠期异常血压及血脂代谢水平与早发型子痫前期明显相关。Objective To investigate blood lipid levels and risk factors in pregnant women with preeclampsia during the second trimester pregnancy. Methodse Serum triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),apolipoprotein(apo) A1,apo B100,free fatty acid(FFA),lipoprotein(a)[Lp(a)] were determined in 456 healthy pregnant women (healthy control group)and 62 pregnant women with preeclampsia at 24-28 gestational weeks. Body mass index (BMI) was calculated at 14-18 gestational weeks. Risk factors were analyzed by multivariate Logistic regression analysis. Results Compared with healthy control group,BMI,systolic blood pressure,diastolic blood pressure,TG,FFA,Lp(a),HDL-C,LDL-C and apo B100 levels were increased in preeclampsia group(P〈0.05),and the difference for other indices between the 2 groups had no statistical significance(P〉0.05). Multivariate Logistic regression analysis showed that after adjusting for age and gestational weeks,BMI at 14-18 gestational weeks [odds ratio(OR)=1.46,95% confidence interval(CI) 1.20-1.78],Lp(a)(OR=1.01,95%CI 1.00-1.02),apo B100(OR=3.27,95% CI 1.07-10.67),systolic blood pressure(OR=4.71,95% CI 2.05-9.32),diastolic blood pressure(OR=3.42,95% CI 1.34-8.64) were risk factors for preeclampsia. Conclusions The abnormal levels of blood pressure and blood lipid metabolism in pregnancy are related to preeclampsia.
