摘  要:目的探讨动脉导管未闭对紫绀型先天性心脏病(CHD)肺血管发育的影响及临床意义.方法选取我院收治的经手术及病理检查确诊为紫绀型CHD患儿36例,将合并动脉导管未闭20例分为研究组,无动脉导管未闭16例分为对照组,观察两组患儿一般资料情况及肺组织病理改变情况.结果两组患儿年龄、红细胞压积、经皮血氧饱和度、心胸比率、血红蛋白水平及肺动脉指数无明显差异(P〉0.05),两组患儿MT、MS、VPSC及MAN等指标比较差异不明显(P〉0.05).结论动脉导管是紫绀型CHD患儿重要的肺血管供血通道,手术中应予以保留,从而确保肺动脉的继续发育,为后续治疗提供良好的条件.Objective To investigate the effect of patent ductus arteriosus on pulmonary vascular development of cyanotic congenital heart disease ( CHD) and its clinical significance. Methods Thirty-six cases of children with cyanotic congenital heart disease confirmed withy operation and pathology were selected as the study objects. The children combine with patent ductus arteriosus were chosen as the study group ( n=20), and the children without patent ductus arteriosus were selected as the control group ( n=16). The general situation and pathological changes of lung tissue of the two groups were observed. Results There was no significant difference on age, hematocrit, percutaneous oxygen saturation, cardiothoracic ratio, hemoglobin level and pul-monary artery index between the two groups ( P〉0.05). There was no significant difference on MT, MS, VPSC and MAN between the two groups ( P〉0.05). Conclusion The ductus arteriosus is an important pulmonary vascular channel in children with cyanotic congenital heart disease. Therefore, it should be preserved during the course of operation, so as to ensure the continuous devel-opment of the pulmonary artery and provide a good prerequisite for the subsequent surgical treatment.
