经会阴超声评估分娩方式对产妇盆底支持组织功能的近期影响

摘　　要：目的探讨经会阴超声评估阴道分娩和选择性剖宫产这两种分娩方式对产妇盆底支持组织功能产生的近期影响.方法选择2013年2月至2015年12月在我院进行分娩的产妇160例,根据产妇分娩方式的差异分为选择性剖宫产组和阴道分娩组,每组80例.采用经会阴超声分别于压力状态下和安静状态下检测两组产妇的膀胱颈距耻骨联合下缘距离（Dr、Ds）、膀胱尿道后角角度（Ar、As）、尿道膀胱连接部移动度（UVJ-M）、膀胱颈旋转角度（0）及肛直肠连接部距耻骨联合下缘距离（ARJ-VD）,比较两组产妇的盆腔器官脱垂发生率.结果与选择性剖宫产组比较,阴道分娩组产妇的Dr、Ds、ARJ-VD均明显降低（P〈0.05）,而Ar、As、UVJ-M均明显增加（P〈0.05）,两组ARJ-VDr相比则无明显差异（P〉0.05）;选择性剖宫产组的盆腔器官脱垂发生率为26.25%（21/80）,明显低于阴道分娩组的63.75%（51/80）（P〈0.05）.结论经会阴超声检查发现,阴道分娩对盆底支持组织功能的近期影响明显大于选择性剖宫产.Objective To investigate the short-term effects of vaginal delivery and elective cesarean delivery on the pelvic floor support function evalued by transperineal sonography. Methods From February 2013 to December 2015, 160 cases of maternal who delivered in our hospital were collected. According to different delivery modes, the women were divided into elective cesarean section group （n=80） and vaginal delivery group （n=80）. Distance from the neck of the bladder to the inferior border of the pubic bone （Dr, Ds）, the angle of the posterior horn （Ar, As）, the mobility of urinary bladderjunction of the urinary bladder （UVJ-M）, the rotation angle of the bladder neck （0） and the distance from the joint of the anus and rectum to the inferior border of the pubic bone （ARJ-VD） of the two groups were detected by perineal ultrasound under pressure and quiet condition. The incidence rates of pelvic organ prolapse in the two groups were compared. Results Compared with the elective cesarean section group, the Dr, Ds, ARJ-VD in vaginal delivery group were obviously lower （P〈0.05）, and Ar, As and UVJ-M were significantly higher （P〈0.05）, there was no significant difference in the resting phase ARJ-VDr between the two groups （P〉0.05）. The incidence rate of pelvic organ prolapse in the elective cesarean section group was lower than that in the vaginal delivery group [26.25%（21/80） vs. 63.75%（51/80）] （P 〈0.05）. Conclusion Through transperineal sonography, the short-term effect of vaginal delivery on pelvic floor support function is significantly greater than that of elective cesarean section.
