兴国县农村居民高血压防治一体化管理效果的评价

摘　　要：目的评价兴国县农村居民高血压防治一体化管理效果。方法选取兴国县试点乡镇的龙口镇和梅窑镇的高血压患者837例,对837例患者实施一体化综合干预[硝苯地平(N)、阿替洛尔(A)、氢氯噻嗪(H)个体化治疗方案]和管理(微机软件系统管理),并在干预前、干预1年后分别进行健康问卷调查,内容包括患者的规律运动、吸烟、饮酒、限盐、高血压健康知识知晓等;体格检查包括血压[收缩压(SBP)、舒张压(DBP)],计算体质指数(BMI)。结果 837例患者干预1年后每天吸烟≥1支且连续吸烟1年以上、每天饮白酒、每天食盐量<6g均较干预前减少,规律运动人数较干预前增加,高血压控制率、规范化管理率、高血压相关健康知识知晓率均显著高于干预前,差异均有统计学意义(均P<0.01)。结论对837例高血压患者实施"预防-诊疗-保健-健康教育-管理"为一体的综合干预有较好的效果,可提高患者的规范化管理率及对高血压相关知识的知晓率。Objective To evaluate the efficacy of integrated management of hypertension prevention and control in rural residents in Xingguo county.Methods A total of 837 hypertension patients from Longkou town and Meiyao town were given integrated intervention(individualized treatment with nifedipine,atenolol and hydrochlorothiazide)and management(computer software management system).The regular exercise,smoking,drinking,salt restriction and hypertension awareness were investigated using the questionnaire before and after intervention for 1year.In addition,the systolic blood pressure(SBP),diastolic blood pressure(DBP)and body mass index(BMI)were measured in patients.Results The numbers of patients who smoked 1cigarette or more every day for 1year,patients who drunk liquor every day and patients who had salt intake<6g every day decreased,and the number of patients who exercised regularly and rates of hypertension control,standardized management and hypertension awareness increased after intervention for 1year(P <0.01).Conclusion The implementation of 'prevention-diagnosis-health carehealth education-management'can effectively improve the rates of standardized management and hypertension awareness in patients with hypertension.
