摘 要:目的:研究活血降糖饮对长期高脂饲料喂养大鼠胰岛β细胞凋亡的影响及其分子机制。方法:采用长期(28周)高脂喂养建立胰岛素抵抗、高脂血症大鼠模型,按随机数字表法分为模型组、中药组、贝特组,另设正常对照组。药物干预治疗后行口服葡萄糖耐量试验(OGTT)及胰岛素释放试验(IRT),并测定各组大鼠生化指标及凋亡相关蛋白表达、胰腺组织病理学及脂肪含量。结果:与模型组比较,中药组大鼠OGTT、IRT明显改善,甘油三酯(TG)、血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDLC)、游离脂肪酸(FFA)明显降低,而高密度脂蛋白胆固醇(HDL-C)明显升高。与正常组比较,模型组大鼠胰腺TG含量明显升高,胰腺可见明显脂肪沉积,胰岛β细胞凋亡明显增加;与模型组比较,中药组和贝特组胰腺脂肪含量明显减少,胰腺脂肪沉积明显改善,胰岛β细胞凋亡明显减少。相关性分析表明,胰岛β细胞凋亡指数与胰腺TG含量呈正相关。与正常组比较,模型组大鼠胰腺Bcl-2蛋白表达明显下调,Bax蛋白表达明显上调,而中药组和贝特组大鼠胰腺Bcl-2蛋白表达明显上调,Bax蛋白表达明显下调。结论:活血降糖饮防治脂毒性所致大鼠胰岛细胞凋亡的作用可能与调节胰腺Bcl-2和Bax蛋白表达有关。Objective: To study the molecular mechanism and effect of Huoxue-Jiangtang prescription on inhibiting pancreatic β cell apoptosis in hyperlipidemia and insulin resistance rats. Methods: High fat diet for 28 weeks, in order to establish the rat model of hyperlipidemia and insulin resistance. The model rats were randomly assigned to model group, the traditional Chinese medicine group (Huoxue-Jiangtang prescription) and fenofibrate group. Corresponding therapy were given through gastric tube for rats in different treatment groups. Meanwhile a control group was set up in normal animals. Oral glucose tolerance test (OGTT) , insulin releasing test (IRT) , serum lipid level, and apoptosis related proteins were detected. Histology and fat content of pancreas were examined, TUNEL and electron microscope were used to evaluate pancreatic g cell apoptosis. Results: In contrast to the control group, the TG content and protein expression of Bax in pancreas tissue were increased, and pancreas fat deposition and early β cell apoptosis were found in model group, Meanwhile, β cell apoptosis index was increased while protein expression of Bcl-2 was reduced in model group. In comparison with model group, the TG content and protein expression of Bax were decreased in pancreas, pancreas fat deposition and cell apoptosis index was decreased in Huoxue-Jiangtaug decoction treated group. Besides, the protein expression of Bcl-2 was increased in Huoxue-Jiangtang decoction treated group. Correlation analysis revealed that β cell apoptosis index was positively correlated with pancreas TG content. Conclusion: It suggested that Huoxue-Jiangtang prescription is effective on treating hyperlipidemia and insulin resistance in rats. The mechanism of preventing β cell apoptosis may be related to the regulation of protein expression of Bcl-2 and Bax in pancreas.
