维持性血液透析患者经络特征及传变规律研究

摘　　要：目的:研究慢性肾脏病维持性血液透析患者的经络特征、体质类型、证候特点及传变规律。方法:采用中医经络检测仪诊断系统于透析前和透析6个月后进行检测维持性血液透析患者,并同期进行中医体质辨识和中医辨证。结果:透析后患者体质以阴虚质、阳虚质、气虚质和瘀血质最为常见,证型主要以脾肾气虚、肝肾阴虚、气阴两虚;透析后患者的自律神经系统、运动状态和体能等检测数据升高,代谢状态、精神状态、检测数据下降。透析前心经、大肠经、小肠经、胃经和膀胱经正值向上,容易患实证;而三焦经、脾经、肾经和肝经负值向下,容易患虚证或慢性病。透析后肺经、心包经和胆经基本处于正常能量区;其他经络处于相对低能量区或相对高能量区。结论:透析后患者向虚性体质转化,脾肾气虚、肝肾阴虚、气阴两虚为主要证型;经络检测结果基本上反映了上述体质和中医证候的变化规律,可为临床提供量化、客观的诊断依据。Objective： To discuss the meridian characteristics, constitutional types, symptoms and transmission principles of patients with chronic kidney disease and maintenance hemodialysis for providing clinical diagnosis and therapy. Methods： Eighty patients who got the first hematodialysis at our hospital were chose, and detection were made by diagnostic system of SHXK-JL-2000 meridian detector before dialysis and after 6 months after dialysis, and at the same term, the recognition of TCM constitution and TCM differentiation were detected. Resutts： Before hemodialysis, static- blood, Yang deficiency and Qi-insufficieney was 20. 0% , 18.8% and 11.3% , and Yin deficiency, Yang deficiency and Qi-insuffieieney was 40% , and phlegm-dampness, dampness-heat, blood stasis and qi depression was 51.3%. After 6 months of hemodialysis, Yin deficiency, Yang deficiency, Qi-insuffieieney and blood stasis were common which were 18.8% , 16.2% , 16.2% and 15.0% , and in which 3 physical qualities of deficiency type were 51.3% , 4 physical qualities of real type were 35.0%. Before dialysis, the main symptom was Yang deficiency （27.5%） , and patients had accompanied symptom and in which dampness, blood stasis and damp and hot were 85%. Deficiency of vital energy of spleen and kidney, deficiency of liver-yin and kidney-yin and deficiency of both qi and yin were the maintain and in which 49 eases had accompanied symptom （61.3%） and dampness, blood stasis and damp and hot were 93.9%. Compared with the data before treatment, patients＇ autonomic nervous system, state of motion and stamina increased, metabolic status, psychosis, test data decreased. Before dialysis, Heart channel, large intestine channel, small intestine channel, stomach channel, and bladder channel were at high energy and sthenia syndrome can be suffered easily. And sanjiao channel, spleen channel, kidney channel and liver channel were downward which can be suffered deficiency syndrome or chronic disease.
