腹主动脉瘤患者高淀粉酶血症的发生及临床分析

摘　　要：目的:探讨腹主动脉瘤(AAA)患者术后高淀粉酶血症的发生率及其与患者预后的相关性。方法:对2015年至2016年,我院综合外科监护室收治的41例危重型AAA患者的临床资料进行回顾性分析。结果:41例危重型AAA患者中术后发生高淀粉酶血证患者27例(65.9%)。以手术方式、动脉瘤是否破裂、术中失血量、术中最低收缩压作为相关因素行Logistic回归发现,手术方式与患者术后的血淀粉酶(AMY)的增高有相关性(P<0.05),行开腹手术患者术后多出现高淀粉酶血症。血淀粉酶的增高程度与患者术后严重不良事件的发生无明显相关(P>0.05)。结论:高淀粉酶血症在危重型AAA的患者中发生率极高,开腹手术的患者更易发生高淀粉酶血症,但不能反应患者术后严重不良事件的发生情况。Objective： To investigate the incidence of postoperative hyperamylasemia in patients with abdominal aortic aneurysm（AAA） and its correlation with the prognosis of patients. Methods： A retrospective analysis was made on the basis of the clinical data of 41 patients with severe AAA admitted in our hospital from 2015 to 2016. Results： The incidence of postoperative hyperamylasemia was found in 27 cases （65.9%） of the total 41 involved patients with abdominal aortic aneurysm. Then, regression analysis was conducted using the operative mode, aneurysm rupture status, intraoperative blood loss and intraoperative lowest systolic blood pressure as relevant factors, corresponding results indicated that there was a correlation between the operative mode and the increased serum amylase （AMY） after operation （ P 〈 0. 05 ） ; Besides, there was a hyperamy- lasemia observed in patients who underwent laparotomy. In addition, there was no significant correlation be- tween the level of serum AMY and the occurrence of severe adverse events after operation （ P 〉 0.05 ）. Conclu- sion： The incidence rate of hyperamylascmia is extremely high in patients with severe AAA, especially higher in patients undergoing laparotomy; However, it cannot predict the incidence of serious adverse events in pa- tients postoperatively.
