摘　　要：目的探讨月经血ADA、TB-Ab水平在女性潜伏性结核患者生殖器中的表达水平。方法将TB-IGRA检测结果阳性、主因原发性输卵管因素行辅助生殖技术助孕的患者作为LTBI组(原发不孕症合并潜伏性结核),将TB-IGRA检测结果阴性、主因男方因素行辅助生殖技术助孕的患者作为对照组。采用ELISA法测定月经血中腺苷脱氨酶(ADA)、TB-Ab浓度,实时荧光定量PCR法测定子宫内膜中结核杆菌DNA含量,同时检测患者外周血CA125、血沉及胸片。比较分析两组月经血腺苷脱氨酶(ADA)、结核抗体(TB-Ab)表达水平。结果LTBI组的ADA、TB-Ab、CA125水平均显著高于对照组,差异有统计学意义;两组的ADA与TB-Ab在月经血中的表达水平呈正相关(r=0.274,P=0.001);LTBI组子宫内膜结核分枝杆菌阳性率很低。结论原发不孕症合并潜伏性结核女性患者月经血中ADA及TB-Ab水平高。潜伏性结核者子宫内膜结核分枝杆菌阳性率很低。Objective To determine concentrations of adenosine deaminase （ADA） and tuberculosis antibody （TB-Ab） in menstrual blood,and mycobacterium tuberculosis DNA concentration in the endometrium of women with primary infertility affected with latent tuberculosis.Methods Patients were divided into two groups by the results of TB-IGRA.The experimental group included patients with TB-IGRA positive results and the main cause of infertility was primary tubal factor （primary infertility with latent tuberculosis infection）.The control group consisted of patients with TB-IGRA negative results and the main cause of infertility was primary male factors.We used enzyme-linked immunosorbent assay （ELISA） to detect the concentrations of ADA and TB-Ab in menstrual blood,and real-time fluorescent quantitative polymerase chain reaction （PCR） to detect mycobacterium tuberculosis DNA content in the endometrium.The patients＆#39;clinical data,such as CA125,blood sedimentation and chest X-ray results were collected.Differences in ADA and TB-Ab levels were compared between the two groups.Results The experimental group＆#39;s ADA,TB-Ab,and CA125 levels were significantly higher than those of the control group （all Ps 〈 0.05）.In both groups,ADA and TB-Ab levels were positive correlated （r =0.274,P =0.001）.Tuberculosis real-time fluorescent quantitative PCR positive rate in the experimental group was very low.Conclusion The expression of ADA and TB-Ab was high in menstrual blood of women with primary infertility affected with latent tuberculosis.However,the positive rate of endometrial tuberculosis was very low.
