超声引导下腔内注射治疗体表静脉畸形的临床疗效分析

摘　　要：目的探讨在超声引导下腔内注射治疗体表静脉畸形的方法和临床疗效。方法在超声引导下采用无水乙醇、平阳霉素局部腔内注射治疗体表静脉性血管畸形,观察瘤体消退及并发症发生情况。疗效评价标准:无效为瘤体无缩小;有效为瘤体缩小40%以下;显效为瘤体缩小41%~80%,色泽接近正常;治愈为瘤体缩小81%~100%,表面色泽正常。结果本组63例患者获随访3~24个月,其中治愈46例(73%),显效12例(19%),有效3例(4%),无效2例(3%)。无一例出现肺纤维化、贫血、肝肾功能损害等并发症。结论超声引导可提高体表静脉畸形腔内注射操作的准确性、安全性及有效性。Objective To explore the application of ultrasound in intratumoral injection treatment of superficial venous malformation.Methods Sixty-three cases were treated by injection of anhydrous alcohol combined with pingyangmycin under the guidance of ultrasound,after which therapeutic efficacy was evaluated and the condition of tumor regression and the incidence of complications was observed.A curative effect evaluation standard was based on improvement in volume,color,and texture after treatment,with the following options：no response-no change was observed in the size or the lesionscontinued to enlarge;improvement-the size of the lesions decreased,but a less than 40% improvement was observed in appearance;marked improvement-the lesions decreased in size more than 41%,but less than 80%,with remarkable improvement in appearance;and cure-the lesions decreased in size more than 81% or disappeared completely without recurrence at least six months after treatment.Results All 63 cases were followed up for 3 to 24 months.Forty-six cases （73%） were cured,12 cases （19.0%） had marked effects,effective in 3 cases （4.0%）,and ineffective in 2 cases （3%）.No functional lesion of liver and kidney,allergy,ardent fever,anemia or pulmonary fibrosis were found in any patients.Conclusions Anhydrous alcohol combined with pingyangmycin intratumoral injection for superficial hemangioma has a high cure rate.Ultrasound guidance improves the safely,accuracy and effectiveness of its operation.
