摘 要:目的探讨结核性渗出性胸膜炎患者血浆降钙素原(PCT)升高的影响因素。方法将2015—2016年西安交通大学医学院附属西安市中心医院收治的30例确诊为结核性渗出性胸膜炎患者作为研究对象,分别于入院当天测定其血浆PCT含量,以PCT≥0.1 ng/ml为研究组,PCT<0.1 ng/ml为对照组,比较两组相关指标(血细胞、胸水生化、血沉、CRP)及是否伴有低氧血症,并行单因素分析,判断是否存在统计学差异,进而确定影响血浆PCT升高的因素。结果30例结核性渗出性胸膜炎患者中,PCT≥0.1 ng/ml有15例(占50%),且同时伴有C反应蛋白升高和低氧血症,与PCT<0.1 ng/ml相比,两组的C反应蛋白、低氧血症具有显著性差异(P<0.05)。结论结核性渗出性胸膜炎患者中C反应蛋白升高、低氧血症是PCT升高的影响因素。Objective To explore the Influencing factors of plasma procalcitonin in patients with tuberculous exudative pleurisy. Methods Selected 30 cases of patients with tuberculous exudative pleurisy, who treated in the pneumology department from January 2015 to June 2016, as a sample. These samples were divided into two groups: the patient with PCT ≥0.1ng/ml were taken in research group and the patient with PCT 〈 0. lng/ml were taken in control group. Compared indicators of each group by using univariate analysis to discover the reasons of level serum procaleitonin in tuberculous exudative pleurisy patients, to determine whether there were significant differences between them and to determine the risk factors of high serum levels of procalcitonin of tuberculous exudative pleurisy patients. Results 30 cases of tuberculous exudative pleurisy patients, PCT ≥0.1ng/ml were in 15 out of 30 patients, elevated CRP and hypoxemia( PaO2〈60mmHg) were causes of high level serum procalcitonin in tuberculous exudative pleurisy patients after using univariate analysis. Compared with PCT≤0.1ng/ml, C reactive protein and hypoxemia in two groups had a significant difference (P〈 0. 05 ). Conclusions Hypoxemia and CRP were the influencing factors of high level serum procalcitonin in tuberculous exudative pleurisy.
