摘  要:目的:分析医院临床致病菌的分布特点及其对常用抗菌药物的耐药性,为临床合理使用抗菌药物提供参考。方法:选取2014年—2016年间临床分离的致病菌,采用全自动微生物分析仪进行菌株鉴定和药敏试验,采用WHONET 5.6版软件对药敏结果进行分析。结果:从各类标本中共分离出致病细菌2 583株,其中大肠埃希菌1 277株占49.44%和肺炎克雷伯菌590株占22.84%,其他致病菌716株占27.72%;大肠埃希菌对哌拉西林耐药率为78.07%,肺炎克雷伯菌对氨苄西林耐药率为63.39%,肠杆菌属对碳青霉烯类、阿米卡星的耐药率较低。结论:医院致病细菌的临床感染以大肠埃希菌和肺炎克雷伯菌为主,其对半合成青霉素的耐药率较高;碳青霉烯类是肠杆菌科细菌最敏感的药物,但也出现了耐碳青霉烯的肠杆菌科菌株;阿米卡星3年来一直保持较低耐药率;应加强对肠杆菌科细菌的耐药性监测,以指导临床合理使用抗菌药物。Objective: To analyze the distribution and antibacterial resistance of 2 583 strains of clinical pathogen- ic bacteria, and to provide a references for clinical rational use of antimicrobial agents. Methods: By selecting clinical isolated pathogenic bacteria during 2014 to 2016, the isolated pathogenic bacteria were authenticated and drug sensitive test using automatic microbial analyzer was conducted. The results were analyzed by WHONET 5.6 software. Results: A total 2 583 isolates of pathogenic bacteria, from various specimens were isolated, Escherichia coli 1 277 isolates were 49.44% and Klebsiella pneumoniae 590 isolates were 22.84%. Other pathogenic bacteria of 716 strains were 27.72%; Escherichia coli to piperacillin resistant rate was 78.07% ; Klebsiella pneumoniae to ampicillin resistant rate was 63.39%. The resistance of Enterobacter Escherichia coli on Carbapenems and Amikacin is the lowest. Conclusion: Escherichia coli and Klebsiella pneumoniae were the raain clinical infections of pathogenic bacteria in our hospital. The resistant rates of them to semisynthetic penicillins were higher. Carbapenems were the most sensitive drugs of entero- bacteriaceae, but there were also carbapenem resistant enterobacteriaceae bacteria. Amikacin maintained a low resis- tance rate for 3 years. The monitoring of drug resistance of enterobacteriaceae should be strengthened, in order to guide clinical rational use of antimicrobial agents.
