乳杆菌活菌胶囊、干扰素α-2b栓联合LEEP术治疗CIN伴HR-HPV的疗效研究

摘　　要：目的探讨乳杆菌活菌胶囊、干扰素α-2b栓联合宫颈环形电切术(LEEP)治疗宫颈上皮内瘤变(CIN)伴高危型人乳头瘤病毒感染(HR-HPV)的疗效。方法选取84例就诊的CIN伴HR-HPV感染患者随机分为观察组和对照组。两组于月经后5~7d行LEEP术,观察组于LEEP术后3d予以乳杆菌活菌胶囊[阴道放置,每次1粒(0.25g),每晚1次]联合干扰素α-2b栓[阴道内放置,每晚1枚(10万IU),隔日1次]治疗;对照组予以单用的干扰素α-2b栓治疗,两组连用3个月经周期。两组术后观察其阴道出血量、创面愈合时间、阴道流液时间及并发症,术后6个月比较CIN治疗效果及HRHPV清除率。结果观察组术后阴道出血量、创面愈合时间及阴道流液时间均少于或短于对照组(P<0.05)。观察组和对照组术后分别出现并发症2例和8例,观察组术后并发症发生率低于对照组(χ~2=4.09,P<0.05)。术后6个月,观察组CIN病变治愈率高于对照组,CIN病变持续或残存率低于对照组(χ~2=3.98,P<0.05),HR-HPV清除率明显高于对照组(χ~2=5.13,P<0.05)。结论乳杆菌活菌胶囊、干扰素α-2b栓联合LEEP术治疗CIN伴HR-HPV感染可促进术后创面的愈合,减少阴道出血量,缩短阴道流液时间,减少术后并发症;并可提高CIN病变治愈率,更有效清除HR-HPV感染。Objective To discuss the effect of live Lactobacillus capsules and interferon α-2b suppository combined with loop electrosurgical excision procedure （LEEP） on cervical intraepithelial neoplasia （CIN） with high risk human papilloma virus （HR-HPV） . Methods Eighty-four cases of CIN with HR-HPV infection were divided into observation group or control group at random. The patients were given LEEP 5 to 7 days after menstruation, followed by intravaginal administration of live Lactobacillus capsules （0.25 g qd at night） and interferon α-2b suppository （100,000 IU qod at night） （observation group） or interferon α-2b suppository alone （control group） 3 days after LEEP for 3 menstrual cycles. The amount of postoperative vaginal bleeding, time to wound healing, days of vaginal fluid and occurrence of complications in both groups were observed, and the cure rates of CIN and clearance rates of HR-HPV were compared 6 monthsr after LEEP. Results The amount of vaginal bleeding, time to wound healing and days of vaginal fluid in observation group were less than in control group （Ps^O. 05） . Two vs eight cases of complications were observed in observation group vs control group respectively after the operation; the difference was significant （χ^2=4.09, P〈0.05） . Compared with the control group, the cure rate of CIN in observation group was higher, while the persistence rate of CIN lesions was lower （χ^2 =3.98, P〈0.05）, and the clearance rate of HR-HPV in observation group was higher than that in control group （χ^2= 5.13, P〈0.05） . Conclusion Live Lactobacillus capsules and interferon α-2b suppository combined with LEEP for treatment of CIN with HR-HPV infection can promote the healing of wound after the operation, reduce the amount of vaginal bleeding, shorten the time of vaginal fluid, reduce the occurrence rate of postoperative complications, improve the cure rate of CIN leisions, and effectively clear HR-HPV infection.
