羟苯磺酸钙胶囊治疗老年糖尿病视网膜病变的疗效及其对VEGF、IGF-1水平的影响

摘　　要：目的探讨羟苯磺酸钙胶囊治疗老年糖尿病视网膜病变的疗效及其对超敏C反应蛋白(hs-CRP)、血管内皮生长因子(VEGF)、胰岛素样生长因子(IGF-1)的影响。方法将68例老年2型糖尿病视网膜病变病人随机分为观察组和对照组,每组各34例。对照组接受基础治疗,观察组在对照组的基础上服用羟苯磺酸钙胶囊,观察2组病人的临床疗效,检测并比较2组病人治疗前后hs-CRP、VEGF、IGF-1水平的变化。结果观察组的总有效率(75.8%)显著高于对照组(21.5%),差异有统计学意义(P<0.05)。治疗后,2组病人的hs-CRP、VEGF、IGF-1水平均较治疗前明显降低(P<0.05),且观察组较对照组降低更显著,差异有统计学意义(P<0.05)。治疗后,观察组Ⅰ期、Ⅱ期病人的hs-CRP、VEGF、IGF-1水平分别低于对照组Ⅰ期、Ⅱ期病人,差异有统计学意义(P<0.05);观察组Ⅲ期病人与对照组Ⅲ期病人相比,hs-CRP、VEGF、IGF-1水平差异无统计学意义(P>0.05)。结论老年糖尿病视网膜病变病人使用羟苯磺酸钙可有效抑制hs-CRP、VEGF及IGF-1水平,进而减轻炎症反应及血管增殖,延缓疾病的进展,提高治疗效果。Objective To investigate the efficacy of calcium dobesilate capsules for elderly patients with diabetic retinopathy（ DR） and the effect on the level of high-sensitivity C-reactive protein（ hs-CRP）,vascular endothelial growth factor（ VEGF） and insulin growth factor-1（ IGF-1）. Methods Sixty-eight elderly patients with DR（ 131 eyes） were randomly divided into observation group and control group,with thirty-four cases in each group. The patients in the control group only received basic treatment,and the patients in the observation group were given calcium dobesilate capsules on the basis of control group. The clinical effects of two groups were observed. The serum levels of hs-CRP,VEGF and IGF-1 were detected and compared between the two groups. Results The total effective rate of the observation group was significantly higher than that of the control group（ 75. 8% vs 21. 5%,P〈 0. 05）.The serum levels of hs-CRP,VEGF and IGF-1 of the two groups after treatment were significantly decreased,especially in observation group（ P〈 0. 05）. After treatment,the levels of hs-CRP,VEGF,IGF-1 of DR patients in the phaseⅠ and Ⅱ of the observation group were significantly lower than those of the patients in the phaseⅠ and Ⅱ of the control group（ P〈0. 05）. There were no significant differences in the levels of hs-CRP,VEGF and IGF-1 between the patients with phase Ⅲ in observation group and control group（ P〉0. 05）. Conclusions Calcium dobesilate capsules can effectively inhibit the levels of hs-CRP,VEGF and IGF-1,reduce the inflammatory reaction and vascular proliferation,which may delay disease progression and improve the efficacy in the elderly patients with DR.
