腹腔镜手术治疗小儿嵌顿性腹股沟斜疝的临床疗效及安全性评价

摘　　要：目的 探讨腹腔镜手术与经腹横纹肌切口术治疗小儿嵌顿性腹股沟斜疝的临床疗效及安全性.方法 选取2014年2月至2015年7月94例嵌顿性腹股沟斜疝患儿,根据手术方式不同分为对照组47例和研究组47例.对照组行经腹横纹肌切口术,研究组行腹腔镜手术.对比两组围术期相关指标水平及并发症发生情况,术后1年随访,统计两组疾病复发率.结果 研究组切口长度、术中出血量、手术用时、术后通气时间、下床活动时间、住院时间均明显优于对照组,差异有统计学意义（P〈0.05）;研究组并发症发生率（12.78%）、疾病复发率（2.13%）明显低于对照组（29.80%、19.15%）,差异有统计学意义（P〈0.05）.结论 采用腹腔镜手术治疗小儿嵌顿性腹股沟斜疝效果显著,创伤小,可减少手术用时及术后机体功能恢复用时,且远期疾病复发率及并发症发生率较低,具有安全性.Objective To investigate the effect and safety of laparoscopic surgery and the abdominal muscle incision in the treatment of pediatric incarcerated inguinal hernia.Methods Ninety-four cases of incarcerated inguinal hernia children were selected from February 2014 to July 2015.According to the different surgical methods, they were divided into control group （n=47） and study group （n=47）.The control group was treated by abdominal muscle incision, the study group was treated by laparoscopic surgery.The perioperative related indicators and the incidence of complications were compared;after 1 year of follow-up, the disease recurrence rate of the two groups were analyzed.Results The length of incision, the amount of bleeding, the time of ventilation, the time of hospital stay and the length of stay in the study group were significantly better than those in the control group, the differences were significant （P〈0.05）;The incidence of complications （12.78%）, disease recurrence rate （2.13%） in the study group were significantly lower than those in the control group （29.80%, 19.15%）, and the differences were significant （P〈0.05）.Conclusions The effect of laparoscopic surgery on children with embedded incarcerated inguinal hernia is significant, the trauma is small, and it can reduce the operation time, postoperative body recovery, and long-term recurrence rate and complication occurred rate is low, and it is safe.
