授精时机对宫腔内人工授精治疗结局的影响

摘　　要：目的:探讨宫腔内人工授精时机对于治疗结局的影响。方法:回顾分析2013年至2015年在我中心第一次行AIH助孕治疗的1127个治疗周期。按照患者行IUI治疗时是否排卵将患者分为排卵前组和排卵后组,比较患者的治疗结局。结果:1127个IUI治疗周期中在排卵前进行手术的有756例,471例患者在确认排卵后接受IUI治疗。其平均年龄、平均不孕年限以及平均的BMI相比均无统计学差异(P>0.05)。两组妊娠率分别为13.10%及11.89%,统计学上差异不明显(P>0.05)。统计学分析显示两组的流产率比较有显著差异,P<0.05,但由于样本例数较少,尚需扩大样本量计算。按照自然周期和促排卵周期分成两个亚组,比较其排卵前后治疗分别的妊娠情况,可以发现无论是自然周期还是促排卵周期两种时机行IUI治疗的妊娠率并没有统计学差异(P>0.05)。结论:在进行有效的IUI治疗时,无论是排卵前后均可以达到相同的妊娠目的。Objectives： To investigate the influence of intrauterine insemination timing on the outcome of intrauterine insemination. Methods： A retrospective analysis was performed on 1127 treatment cycles of AIH assisted pregnancy for the first time in our center from 2013 to 2015. According to the existence of ovulation, the patients were divided into before ovulation and after ovulation group. The treatment outcome of the two groups was compared. Results： There were 756 patients received IUI treatment before ovulation, and 471 patients received IUI treatment after ovulation. There was no significant difference in the average age, average age of infertility and average BMI （P 〉0. 05）. The pregnancy rates of the two groups were 13. 10% and 11.89% respectively, without statistically significant difference （ P 〉 0. 05 ）. Statistics analysis showed that difference in the abortion rate between the two groups was significant （ P 〈 0. 05 ） , but because of the small sample size, more patients should be included. Pa- tients were further divided into natural ovulation subgroup and ovulation induction group, and no significant differ ence was found in the pregnancy rate between the two groups （ P 〉 0. 05 ）. Conclusion： Effective IUI treatment, whether before or after ovulation, can achieve equivalent pregnancy outcome.
