双球囊导管在足月妊娠促宫颈成熟及引产应用方面联合观察研究

摘　　要：目的:探讨双球囊导管在妇科引产中的有效性、安全性。方法:随机选取2015年2月至2016年2月入余姚市人民医院分娩的600例产妇,分成A、B、C组。A组为使用双球囊导管引产产妇153例,B组为药物引产产妇(控释地诺前列酮栓)147例,C组为自然分娩产妇300例(作为对照)。用统计学方法对上述数据进行分析,并用Excel表格模拟各种曲线。结果:A组放置导管前后宫颈评分有所提高,A组剖宫产的比率明显下降,并且因为引产失败等引起的剖宫产明显少于B组。在引产安全性上A组较高。在产程上,A组较B组第一产程潜伏期短,活跃期相对长。两组间比较差异有统计学意义(P<0.05)。结论:双球囊导管在足月妊娠促宫颈成熟及引产方面,效果良好,副作用小,值得在妇科临床上大力推广使用。Objectives： To explore the efficacy and safety of the double balloon catheter in teruls of Gynecological abortion. Methods： 600 puerperae in our hospital from February 2015 to February 2016 were selected and randomly divide into group A, B and C. The 153 puerperae in group A adopted double balloon catheter for induced labor; the 147 puerperae in the group B used medicine （controlled- release dinoprostone suppository） for induced labor; the 300 puerperae in the group C adopted natural delivery （as control）. Statistical methods were used to analyze the data and Excel spreadsheet was used to simulate various curves. Results： The cervical scores of group A were enhanced after placing the catheter, and the incidence of cesarean section decreased, which was significantly lower than group B. Group A was of high safety in induced labor. In terms of stages of labor, the first stage incuba- tion period of group A was shorter than group B, with longer active period. The difference between the two groups was statistically significant （P 〈 0. 05）. Conclusion： Double balloon catheter is effective in cervical ripening and induction of labor for full - term pregnancy with few side - effects, which is worth promoting in clinical.
