“三维递进式”疗法干预早期糖尿病肾病疗效观察

摘　　要：目的:观察"三维递进式"疗法对早期糖尿病肾病气阴两虚型患者的短期疗效,探讨该疗法的作用机理。方法:选取早期糖尿病肾病气阴两虚型患者120例,采用随机数字表分为A组、B组和C组各40例。在常规治疗和护理的基础上,A组予保留灌肠,B组予保留灌肠和熏洗双足治疗,C组予"三维递进式"疗法治疗。所有患者干预2个周期,6d/周期,治疗后观察3组总体疗效、中医证候疗效,mAlb、HbA1c水平变化情况及不良事件发生情况。结果:研究期间5例脱落,115例患者完成研究,3组治疗期间均未发生重度不良事件。3组临床总有效率组间差异无统计学意义(P>0.05);C组中医证候总有效率和m Alb值水平优于B组、A组,差异有统计学意义(P<0.05);3组HbA1c水平组间差异无统计学意义(P>0.05)。结论:在早期糖尿病肾病应用"三维递进式"疗法,能提高中医证候疗效,有效降低mAlb排泄,且无明显不良反应,值得临床推广应用。Objective To observe the shortterm effects of ＂Three -Dimensional progressing therapy＂ for early -stage Diabetic Nephropathy patients which deficiency of both qi and yin and exploring the mechanism of this therapy. Methods 120 patients with early stage DN were divided into A group, B group and C group. On the basis of routine treatment and care, A group was treated with reten- tion enema, B group was treated with retention enema and feet fumigation, and the C group was treated with ＂Three - Dimensional pro- gressive therapy＂. All patients acquire 2 courses treatment and one course include six days. After treatment, the overall curative effect of three groups, the efficacy of TCM syndrome, the changes of mAlb and HbA1 c and the occurrence of adverse events were observed. Results During the study period, 5 cases fell off and 115 patients completed the study. No serious adverse events occurred during the three groups. There was no significant difference between the three groups in the total effective rate （ P 〉 0. 05 ）. There was no signifi- cant difference in HbA1 c level between the three groups （P 〈 0. 05 ）. The concentration of mAlb and TCM symptom scores in group C was better than that in group B and group A （ P 〈 0. 05 ）. Conclusion The ＂three - dimensional progressing therapy＂ was worthy ap- plication in the early- stage diabetic nephropathy of clinical because it can improve the efficacy of TCM syndromes, effectively reduce mAlb excretion, and no obvious adverse reactions were happened during the courses.
