摘 要:目的全面掌握咸宁市人禽流感病毒感染状况,进一步加强该市人感染H7N9禽流感防控工作,了解此病毒活动强度,为全面评估防控效果提供有效依据。方法选择2015年4—7月咸宁市所有二级以上医疗机构门(急)诊就诊符合流感样病例的所有患者及病房符合住院监测病例定义的所有患者,对其进行甲型流感病毒抗原快检,将结果为阳性的咽拭子标本送至咸宁市疾病预防控制中心流感监测网络实验室,采用实时荧光定量聚合酶链反应(Real-Time PCR)检测方法进行流感病毒核酸检测分型,并对检测的结果进行统计学分析。结果咸宁市二级以上医疗机构开展甲型流感病毒快检2 782人次,快检阳性标本76例,经咸宁市流感监测国家网络实验室核酸检测阳性标本53例,均为甲型季节性H3亚型。结论在为期3个月的人感染H7N9禽流感强化监测中,由于监测力度和范围的扩大,经过医疗机构的快检筛查,流感核酸检测阳性率明显高于常规监测。该措施在防控工作中为尽早发现及诊断人感染H7N9流感病例提供了重要依据。[ Objective ] To fully monitor the situation of human infection with avian influenza A ( H7N9 ) in Xianning City, further strengthen the prevention and control work, understand the activity of infection with avian influenza A (H7N9), and provide the effective basis for the comprehensive assessment of prevention and control effect. [ Methods ] All outpatients of influenza like illness, as well as all inpatients met hospitalized monitoring standard in secondary or above medical institution of Xianning City from April to July in 2015 were recruited in the study. The rapid detection of influenza A virus antigen was performed, and the positive throat swab samples were sent to the Influenza Surveillance Network Laboratory of the Xianning Center for Disease Control and Prevention to carry out the detection and typing of influenza virus nucleic acid by the method Real-Time PCR, and the results were statistically analyzed. [ Results ] The rapid detection of influenza A virus antigen was conducted in 2 782 cases from secondary or above medical institution of Xianning City, 76 samples were positive. Totally 53 cases were confirmed with positive virus nucleic acid by the National Influenza Surveillance Network Laboratory, and all of them were seasonal influenza A subtype H3. [Conclusion] During the three months of intensive monitoring for human infection with avian influenza A (HTN9), due to the expansion of monitoring intensity and scope, the positive rate of influenza A virus nucleic acid in the rapid screening of medical institutions is significantly higher than that of routine surveillance. This measure provides important evidence for the early discovery and diagnosis of human infection with avian influenza A (H7N9) in the prevention and control work.
