老年性白内障围手术期眼内炎的预防及易感因素分析

摘　　要：目的探讨老年性白内障术后眼内炎感染的临床特点及不同预防措施的预防效果。方法选取2010年2月-2016年3月78例老年白内障术后发生眼内炎患者为病例组,另选78例老年白内障术后未发生眼内炎及其他并发症的患者为对照组,对比分析两组患者术前临床资料,观察白内障术后不同预防措施的预防效果。结果病例组合并高血压、糖尿病的患者高于对照组(P<0.05);患者术后发生眼内炎中,表皮葡萄球菌感染率较高,感染率为76.92%,霉菌感染为10.26%,不同感染采用不同治疗方式对术后视力的恢复有重要意义。结论老年性白内障术后眼内炎是白内障术后严重的并发症,早期进行病原学检查和积极做好术前、术中眼内炎的预防,一旦发生白内障术后眼内炎,要根据病原菌特点采取不同治疗措施,有助于视功能的恢复。OBJECTIVE To explore the clinical characteristics of postoperative endophthalmitis in patients with senile cataract and observe the effects of the prevention measures.METHODS Totally 78 senile cataract patients with postoperative endophthalmitis who were treated in hospitals from Feb 2010 to Mar 2016 were chosen as the case group,meanwhile,78 senile cataract patients without postoperative endophthalmitis or complications were set as the control group.The preoperative clinical data were analyzed and compared between the two groups of patients,and the effects of the prevention measures were observed.RESULTS The proportion of the patients complicated with hypertension,diabetic mellitus was significantly higher in the case group than in the control group （P〈0.05）.Among the patients with postoperative endophthalmitis,the incidence rate of Staphylococcus epidermidis infections was 76.92%,and the incidence rate of mycotic infection was 10.26%.It was of great significance to treat the infections by different measures so as to facilitate the recovery of postoperative vision.CONCLUSION The postoperative endophthalmitis is a severe complication in the patients with senile cataract.It is necessary to conduct etiological examination in early stage,take prevention measures for preoperative,intraoperative endophthalmitis,and adopt different treatment measures on the basis of characteristics of pathogens once the postoperative endophthalmitis occurs so as to facilitate the recovery of visual function.
