后腹腔镜下输尿管切开取石术与输尿管镜钬激光碎石治疗输尿管上段结石的疗效比较

摘　　要：目的比较分析后腹腔镜下输尿管切开取石术与输尿管镜钬激光碎石治疗输尿管上段结石的疗效。方法选取在2014年1月至2015年12月在昆山市第一人民医院进行治疗的单侧输尿管上段结石患者70例,按手术方式不同分为两组,各35例。一组实施后腹腔镜下输尿管切开取石术(RLU),一组采用输尿管镜钬激光碎石(URL),比较两组患者结石清除率、手术时间、住院时间及手术并发症。结果 RLU组结石清除率100%,手术时间(70.2±24.8)min,住院时间(13.5±4.8)d,手术并发症率0%,URL组结石清除率82.8%,手术时间(35.4±14.6)min,住院时间(9.4±3.6)d,手术并发症率2.8%,RLU组住院时间及手术时间均长于RUL组,具有统计学意义(P<0.05),RLU结石清除率高于RUL组,两组术后并发症率均无明显统计学差异。结论后腹腔镜输尿管切开取石术及输尿管镜钬激光碎石均可用于治疗输尿管上段结石,且安全、有效,而RUL清石率更高,术者可以从自己的技术熟练程度及患者的情况选择手术方式。Objective To compare the efficacy ofuretrolithotomy under retroperitouneal la-paroscopy and ureteroscopic holmium laser lithotripsy in the treatment of upper ureteral calculi. Methods 70 patients with upper ureteral calculi were admitted to fist people＇ Hospital of kunshan city from January 2014 to December 2015, according to the different surgical methods were divided into two groups, each of 35 cases. A group of ureterolithotomy under retroperitoneal la-paroscopy （RLU） , a group of ureteroscopic holmium laser lithotripsy （ URL ） , compartwo groups of patients with calculi clearance rate, operation time, The length of stay and surgical complications. Results in RLU group, the calculi clearance rate was 100%, surgery time was （ 70.2 ± 24.8 ） rain, length of stay was （ 13.5 ± 4.8 ） d, Surgical complications rate was 0%, in URL group, the calculi clearance rate was 82.8%, surgery time was （ 35.4 ± 14.6 ） min, length of stay was （ 9.4 ± 3.6 ） d, Surgical complications rate was 2.8%. The surgery time and the length of stay in RLU group were both longer than those in RUL group, and the differences were Statistically significant, the calculi clearance rate in RLU group was higher than that in RUL group, the Surgical complications rate of two groups were no Statistically significant .Conclusiou ureterolithotomy under retroperitoneal la-paroscopy and ureteroscopic holmium laser lithotripsy can be used for the treatment of upper ureteral calculi, they are both safe and effective, RUL has a higher the calculi clearance rate. The surgeon can choose the operation mode from the technical proficiency and the patient＇ s condition.
