摘　　要：目的：探讨不同固定方式对C型桡骨远端骨折术后长度维持以及腕关节功能的影响.方法：选取2013年5月～2014年12月我院收治的C型桡骨远端骨折患者74例为研究对象,采用随机数表法分为观察组和对照组各37例,对照组实施外固定架固定术,观察组实施掌侧锁定钢板固定内固定术,比较2组桡骨改善优良率,采用Cooney腕关节评分法比较2组腕关节功能,同时测量其桡骨远端长度维持程度,并观察并发症.结果：观察组治疗优良率91.89％,高于对照组72.97％（P〈0.05）;观察组桡骨远端长度丧失0.92±0.58mm,低于对照组3.10±0.14mm,观察组Conney评分90.04±1.25分,高于对照组78.36±1.13分（P〈0.05）;2组并发症发生率5.40％、8.11％,比较无显著差异（P〉0.05）.结论：掌侧锁定钢板固定内固定术对C型桡骨远端骨折治疗效果较外固定支架固定术好,能明显维持桡骨长度,改善患者腕关节功能,值得在临床推广应用.Objective： To investigate the effect of different fixation methods on length maintenance and wrist joint function after surgery of type C distal radius fractures. Methods： Seventy-four patients with type C distal radius fractures admitted in our hospital between May 2013 and December 2014 were randomized into observation group and control group by the random number table method, 37 cases in each group. The control group was treated by external fixation, while the observation group was treated by internal fixation with volar locking plate. The excellent rate of radial improvement was compared between the two groups. The wrist joint function was compared by Cooney wrist score. The degree of length maintenance of the distal radius was measured, and the complications were observed. Results： The excellent rate of treatment in the observation group （91.89％） was higher than that in the control group （72.97％） （P〈0.05）; The length loss of distal radius in the observation group [（0.92±0.58） mm] was lower than that in control group [（3.10±0.14） mm] while Conney score （90.04±1.25） was higher than that in the control group （78.36±1.13） （P〈0.05）; There was no significant difference in the incidence of complications between the two groups （5.40％ vs 8.11％） （P〉0.05）. Conclusion; The effect of internal fixation with volar locking plate is better than that of external fixation in the treatment of type C distal radial fractures. The former can significantly maintain the radial length and improve the wrist joint function of patients. It is worthy of clinical application.
