摘　　要：目的：比较老年股骨粗隆间骨折采用DHS和PFNA固定的临床治疗效果.方法：选择我院2007年8月～2016年5月收治的48例老年股骨粗隆间骨折患者,根据患者固定方法的不同分为DHS组和PFNA组,比较2组患者的手术时间、术中出血量、骨性愈合时间、临床治疗效果及并发症情况.结果：PFNA组患者平均手术时间82.17±12.30分钟,术中平均出血量293.52±32.18ml,骨性愈合时间8.25±1.43周,PFNA组患者的手术时间、术中出血量和骨性愈合时间显著低于DHS组（P〈0.05）,具有统计学意义;DHS组治疗总有效率为79.17％,并发症发生率为25.00％,PFNA组治疗总有效率为91.67％,并发症发生率为8.83％,2组患者在临床治疗效果及并发症方面存在显著性差异（P〈0.05）,具有统计学意义.结论：DHS和PFNA固定治疗老年股骨粗隆间骨折治疗均具有明显的治疗效果,PFNA固定治疗与DHS相比缩短了手术时间、减少术中出血量,临床治疗效果显著优于DHS固定.Objective： To compare the old age femoral intertrochanteric fracture with DHS and PFNA fixed clinical therapeutic effect. Methods： From August 2007 to May 2016 of 48 cases of elderly patients with femoral intertrochanteric fracture, depending on the patients with fixed method into the DHS group and PFNA group, compared two groups of patients with operation time, intraoperative blood loss, osseous healing time, clinical therapeutic effect and complications. Results： Patients with PFNA group, the average operation time（82.17＋12.30）min, the average intraoperative blood loss （293.52＋32.18）ml, osseous healing time （8.25 ＋ 1.43）w, PFNA group of patients with operation time, intraoperative blood loss and osseous healing time was significantly lower than DHS group（P〈0.05）,statistically significant; DHS treatment group the total effective rate was79.17％,the incidence of complications was25.00％,PFNA treatment group total effective rate was 91.67％,the incidence of complications was8.83％,two groups of patients in the clinical treatment effect and complications have significant difference （P〈0.05）,with statistical significance. Conclusion： DHS and PFNA fixation for senile femoral intertrochanteric fracture treatment has obvious effect, fixed PFNA treatment compared with DHS to shorten the operation time, reduce intraoperative blood loss, clinical treatment effect is significantly better than that of DHS fixation.
