摘 要:目的:用生物标志物预测雄激素性秃发(AGA)患者的疗效。方法:307例AGA患者在脱发边缘区拔取生长期毛发,测量磺基转移酶(SULT 1A1)活性[用吸光度(A)值表示],并询问患者年龄、脱发家族史及体重指数(BMI)。予以2%米诺地尔外用,每日2次,每次1 mL,连续治疗6个月。在基线、治疗6个月时进行全局照相评估,根据患者治疗前后脱发面积和程度的变化判断疗效,将患者分为3组,无效组为病情未控制,脱发继续加重;控制组为病情得到控制;改善组为毛发有再生。结果:治疗6个月后,每例患者测量4根毛发时,改善组与控制组、无效组之间A值差异有统计学意义,控制组与无效组之间A值差异无统计学意义(P_(10)=0.000,P_(1-1)=0.028,P_(0-1)=0.702);每例患者测量8根毛发时,改善组与控制组、无效组之间A值差异有统计学意义,控制组与无效组之间A值差异无统计学意义(P_(10)=0.000,P_(1-1)=0.021,P_(0-1)=0.775)。不同疗效积分之间的A值差异无统计学意义(P>0.05)。A值与性别、年龄、脱发家族史、BMI、就诊时脱发严重程度和拔取毛发的部位无相关性。结论:检测SULT 1A1活性可以预测米诺地尔的疗效,4根毛发的敏感性为83%,特异性为82.6%;8根毛发的敏感性为83%,特异性为87%。Objective: Topical minoxidil is the most common drug used for the treatment of androgenetic alopecia(AGA) in men and women. Although topical minoxidil exhibits a good safety profile, the efficacy remains relatively low. So a biomarker for predicting patient response prior to therapy would be advantageous. Methods: Three hundred and seven AGA patients were plucked anagen hair from the border between bald and non-bald scalp, measured sulfotransferase(SULT 1A1) activity(A), and documented age, BMI, the family history of alopeeia. All patients were given 1 mL 2% topical minoxidil solution respectively, twice a day for 6 months. Global photography was performed at baseline and 6 months in order to evaluate the drug efficacy, according to the changes of areas and degrees of hair loss before and after treatment. Results: After treatment for 6 months, A of 4 hairs were differenees in the size between the improved group and the controlled group (P=0.000). They were differences in the size between the improved group and the ineffective group (P=0.028). They were no differences in the size between the controlled group and the ineffective group(P〉0.05). A of 8 hairs were differences in the size between the improved group and the controlled group(P= 0.000). They were differences in the size between the improved group and the ineffective group(P=0.021). They were no differences in the size between the controlled group and the ineffective group(P〉0.05). There was no significant difference in A among the efficacy scores of 50, 100, 150. A were not affect sex, age, BMI, the family history of alopecia, the severity of alopecia during the patient's first visit and the part of plucked hairs. Conclusions: A of 4 hairs was able to predict responders to minoxidil therapy with a sensitivity of 83% and a specificity of 82.6%. A of 8 hairs was a sensitivity of 83% and a specificity of 87%.
