摘  要:目的分析经皮冠状动脉介入(PCI)术后患者血清超敏C反应蛋白(hs-CRP)、血清淀粉样蛋白A(S从)与冠状动脉支架内再狭窄(ISR)的关系。方法选取2015年4月—2016年4月武汉大学人民医院心内科经冠状动脉造影诊断为冠心病并行PCI术的患者121例作为研究对象,根据Gensini积分标准对患者冠状动脉病变进行评分,复查冠状动脉造影,并根据是否存在支架血管直径狭窄≥50%分为再狭窄组(ISR组)34例和非再狭窄组(NISR组)87例,比较2组患者一般资料、实验室指标及冠状动脉病变情况,分析ISR发生的危险因素。结果 2组患者一般资料(性别、年龄、合并症等)、冠状动脉病变部位、数量和程度差异无统计学意义(P>0.05),ISR组术后hs-CRP、SAA升高明显高于NISR组(t=3.825、3.723,P<0.05),其余实验室指标差异无统计学意义(P>0.05)。Logistic回归分析表明高水平的hs-CRP、SAA是ISR的独立危险因素(OR=1.221、1.058,P<0.05)。结论 PCI术后hsCRP、SAA水平越高,发生再狭窄的风险越大,高水平的hs-CRP、SAA可能是ISR的独立危险因素。Objective To explore the associations of serum high sensitivity C reactive protein (hs-CRP) and serum amyloid A (SAA) concentrations after PCI with in-stent restenosis (ISR).Methods One hundred and twenty-one patients who were diagnosed CHD by coronary angiography and underwent PCI treatment from April 2015 to April 2016 in Renmin Hospital of Wuhan University were enrolled.We evaluated every patient's coronary lesions according to Gensini score criteria.Patients who were followed up 8 months after PCI were divided into ISR group (34 cases) or NISR group (87 cases) according to whether combined with ISR (The ISR was defined as the reduction of lumen diameter≥50%).The basic clinical data, laboratory results and coronary lesions of the patients in two groups were compared.Results There were no significant difference in the basic clinical data, location, number and degree of coronary lesion between the two groups (P〉0.05).Compared with NISR group, ISR group was significantly higher in level of hs-CRP and SAA(t=3.825,3.723,P〈0.05), but other laboratory results have no significant difference between the two groups (P〉0.05).The logistic analysis showed that a relatively higher level of hs-CRP and SAA (OR=1.221,1.058,P〈0.05) may be the independent risk factors of ISR.Conclusion The higher the level of hs-CRP and SAA after PCI, the greater the risk of ISR.The high level of hs-CRP and SAA may be the independent risk factors of ISR.
