组织多普勒对应用ACEI高血压患者左室舒张功能改善程度的评估价值

摘　　要：目的:分析组织多普勒评估血管紧张素转化酶抑制剂(angiotensin converting enzyme inhibitors,ACEI)对改善高血压患者左室舒张功能的影响。方法:选取43例高血压患者作为研究对象,分为左室肥厚组和非左室肥厚组,均给予ACEI治疗,对比治疗前后使用组织多普勒测量二尖瓣环舒张早期运动速度(Ve)、晚期运动速度(Va)、Ve/Va,并与血流多普勒指标二尖瓣口舒张早期充盈速度(E)、晚期充盈速度(A)、E/A进行比较,观察降压效果和治疗前后组织多普勒、血流多普勒指标的改善程度。结果:左室肥厚组Ve、Ve/Va均低于非左室肥厚组(P<0.05);左室肥厚组与非左室肥厚组间的E、A、E/A差异无统计学意义(P>0.05);经ACEI治疗结束后,收缩压、舒张压均较治疗前降低(P<0.05),但心率无明显变化(P>0.05);与治疗前相比,治疗后E、E/A、Ve、Ve/Va升高(P<0.05),而Va、A治疗前后差异无统计学意义(P>0.05)。结论:组织多普勒可作为评价左室舒张功能的可靠方法,可评估ACEI高血压患者的左室舒张功能,较血流多普勒更准确。Objective： To analyze the clinical value of angiotensin converting enzyme inhibitors（ ACEI） on the improvement of left ventricular diastolic function in patients with hypertension by tissue Doppler imaging（ TDI）. Methods： 43 patients with hypertension were collected and divided into the LVH group and the non-LVH group,treated with ACEI. The indexes of early diastolic velocity of mitral annulus（ Ve）,late diastolic velocity of mitral annulus（ Va） and Ve/Va were detected by TDI and compared before and after the treatment. And the indexes of early left ventricular diastolic velocity（ E）,atrial systole velocity（ A） and E/A were detected by color Doppler flow imaging（ CDFI） and compared with the above indexes by tissue doppler imaging（ TDI）. The hypotensive effect and the improvement degrees of the indexes by TDI and CDFI observed. Results： The indexes of Ve and Ve/Va in LVH group were lower than those in the non-LVH group（ P〈0. 05）. There was no difference in terms of the indexes of E,A and E/A between the two groups（ P〉0. 05）. After the antihypertensive therapy by ACEI,the systolic pressure and diastolic pressure were lower than those before the therapy（ P〈0. 05）,but there was no difference in terms of the heart rate before and after the treatment（ P〉0. 05）. The indexes of E,E/A,Ve and Ve/Va after the treatment were higher than those before the treatment（ P〈0. 05）. But there was no difference in terms of indexes of Va and A before and after the treatment（ P〉0. 05）. Conclusion： TDI is reliable in evaluating the left ventricular diastolic function and can be used to assess the left ventricular diastolic function of patients with hypertension treated by AECI,which is more accurate than CDFI.
