血清胱抑素C与其它肾功能指标对早期肾功能损害的临床诊断价值

摘　　要：目的:探讨血清胱抑素C与其它肾功能指标对早期肾功能损害的临床诊断价值。方法:选取确诊有早期肾功能损害的患者273例为观察组,选取同期肾功能正常的健康体检者210例为对照组。检测所有研究对象的血尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,Scr)、血清β2-微球蛋白(β2-microspheres,β2-MG)和血清胱抑素C(cystatin C,Cys-C)。观察对比肾功能正常人群与早期肾功能损害患者的BUN、Scr、β2-MG及Cys-C;采用受试者工作特征(receiver operating characteristic,ROC)曲线分析BUN、Scr、β2-MG及Cys-C各指标单用或联用对早期肾功能损害程度的诊断价值。结果:观察组的BUN、Scr、β2-MG及Cys-C含量均高于对照组(P<0.05);ROC曲线的曲线下方面积(area under the curve,AUC)显示:Cys-C的AUC为0.823,高于BUN的0.772、Scr的0.781和β2-MG的0.819。各指标联用评估早期肾功能损害的AUC(0.905)均高于其单用。结论:BUN、Scr、β2-MG及Cys-C各指标联用对早期肾功能损害程度的临床诊断价值较满意,具有重要的临床应用价值。Objective： To explore the clinical diagnostic value of serum cystatin C and renal function indexes in evaluating the early renal damage. Methods： 273 patients with early renal function impairment were chosen as the observation group,and 210 healthy subjects with normal renal function were chosen as the control group. The blood urea nitrogen（ BUN）,serum creatinine（ Scr）,serum β2-microglobuli（ β2-MG） and serum cystatin C（ Cys-C） were detected. The BUN,Scr,β2-MG and Cys-C in early renal damage patients and normal renal function population were observed and compared. The diagnostic value that was used alone or in combination with BUN,Scr β2-MG and Cys-C of early renal function damage were analyzed by receiver operating characteristic（ ROC） curve. Results： The BUN,Scr,β2-MG and Cys-C in the observation group were higher than those in the control group,the differences being statistically significant（ P〈0. 05）. ROC curve showed that the AUC（ 0. 823） of Cys-C was higher than BUN（ 0. 772）,Scr（ 0. 781） and β2-MG（ 0. 819）. The AUC（ 0. 905） of combined indexes used to evaluate early renal damage were higher than those of single use. Conclusion： The clinical diagnostic value of combined indexes（ BUN,Scr,β2-MG and Cys-C） used to evaluate early renal damage is satisfactory,which has important clinical value.
