摘 要:目的:探讨肺间质纤维化合并肺气肿(combined pulmonary fibrosis and emphysema,CPFE)患者血清中转化生长因子β1(transforming growth factor-β1,TGF-β1)、血管内皮生长因子(vascular endothelial growth factor,VEGF)及胰岛素生长因子-Ⅰ(insulin-like growth factor-1,IGF-Ⅰ)的表达变化。方法:选取CPFE患者40例、肺气肿患者40例以及特发性肺间质纤维化(idiopathic pulmonary fibrosis,IPF)患者40例,分别作为CPFE组、肺气肿组和IPF组。采用ELISA法检测3组患者血清TGF-β1、VEGF及IGF-Ⅰ水平。结果:(1)CPFE组和IPF组血清TGF-β1水平均高于肺气肿组(P<0.05),CPFE组和IPF组血清IGF-Ⅰ水平均高于肺气肿组(P<0.05),IPF组患者血清VEGF水平分别高于肺气肿组和CPFE组(P<0.05),但CPFE组与肺气肿组比较,差异无统计学意义(t=1.79,P>0.05);(2)CPFE组吸烟患者血清VEGF水平低于非吸烟者(P<0.05);(3)CPFE组和肺气肿组血清TGF-β1水平与IGF-Ⅰ水平均呈正相关(P<0.05)。结论:CPFE的发生、发展可能与血清TGF-β1、IGF-Ⅰ及VEGF水平的变化存在一定联系,检测其血清水平可为CPFE的诊断与治疗提供一定的参考依据。Objective: To investigate the changes of expressions of serum transforming growth factor-β1( TGF-β1),vascular endothelial growth factor( VEGF)and insulin-like growth factor-1( IGF-Ⅰ) in patients with pulmonary fibrosis combined emphysema. Methods: 40 patients with CPFE,40 patients with emphysema and 40 patients with idiopathic pulmonary fibrosis( IPF) were selected as the CPFE group,the emphysema group and the IPF group respectively. ELISA method was used to detect the levels of serum TGF-1,VEGF and IGF-Ⅰ in the patients of the three groups. Results:( 1) The levels of serum TGF-β1 in the CPFE group and the IPF group were higher than that of the emphysema group( P〈0. 05). The levels of serum IGF-Ⅰ in the CPFE group and the IPF group were higher than thatinthe emphysema group( P〈0. 05). The levels of serum VEGF in the IPF group was higher than those inthe emphysema group and the CPFE group respectively( P〈0. 05),but there was no significant difference between the CPFE group and the emphysema group( t = 1. 79,P〉0. 05);( 2) The level of serum VEGF of smokers in the CPFE group was lower than that of non-smokers( P〈0. 05);( 3) The levels of serum TGF-β1 and the levels of IGF-Ⅰ in the CPFE group and the emphysema group were positively correlated( P〈0. 01). Conclusion: The occurrence and development of CPFE may be related to the changes of serum TGF--β1,IGF-Ⅰ and VEGF. The detection of serum levels may provide a certain reference basis for the diagnosis and treatment of the disease.
