剖宫产术应用舒芬太尼对新生儿的影响

摘  要:(1)目的研究舒芬太尼静脉注射用于剖宫产时对新生儿的影响。(2)方法择期行剖宫产术的健康孕妇40例,随机分为两组。舒芬太尼组(S组)于胎儿娩出前10分钟静脉给予舒芬太尼0.15μg/kg,而对照组(C组)给予相同体积的生理盐水2mL。记录产妇术中生命体征、不良反应、视觉模拟疼痛评分(VAS)及镇静评分(Ramsay),新生儿1、5、10分钟Apgar评分、脐动脉血气和2、24小时神经行为评分(NBNA)。(3)结果与C组和入室平静时基础值比较,S组在切皮时、胎儿娩出时产妇镇静评分升高(P<0.05),视觉模拟疼痛评分减低(P<0.05);两组各时间点Apgar评分、NBNA评分、不良反应评分差异无统计学意义(P>0.05)。(4)结论剖宫产手术时,母体应用小剂量舒芬太尼能增强麻醉镇痛镇静效果,且对新生儿及母体无明显影响。Objetive To study effects of intravenous sufentanil on neonate in cesarean delivery. Methods Healthy parturient women underwent elective cesarean section ( n =40) were randomly divided into two groups:group S (sufentail group) received 0.15μg/kg intravenous sufentanil at 10 minutes before fetal childbirth;group C (contral group) received 2mL 0.9%N.S.And record maternal vital signs,adverse reactions, visual analog pain score (VAS) and sedation scores (Ramsay) during operation.Neonatal assessment included Apgar scores at l min,5min and 10rain after delivery, umbilical arterial blood gas analyses, and neurobehavioral testing at 2h and 24h of life using the neonatal behavioral neurological assessment (NBNA).Results The sedation scores in group S was higher than in group C at the time of skin incision and delivery( P 〈0.05),while the scores of VAS was lower ( P 〈0.05). There were no significant differences in Apgar scores, NBNA and adverse reactions in different time between the groups (P 〈 0.05). Conclusion In cesarean section, maternal application of low-dose sufentanil can enhance analgesic and sedative effects,and no obvious adverse effects on the mother and newborn was found.
