摘 要:目的:探讨正弦图确定迭代重建技术(SAFIRE)在肝脏低千伏CT增强扫描中的应用价值。方法:选择临床怀疑肝脏疾病的54例患者分为A、B两组,行上腹部CT增强扫描,A组管电压为120 kVp、使用滤波反投影(FBP)技术进行图像重建,B组管电压为80 kVp、使用SAFIRE技术进行图像重建,强度为3级;比较两组患者的CT扫描有效剂量(ED),采用客观评价指标[肝实质CT值、图像噪声、对比噪声比(CNR)及信噪比(SNR)]及主观评价指标(图像噪声、血管显示、伪影及图像整体质量)评价A、B两种CT增强扫描方法的图像质量。结果:与A组相比,B组有效剂量减少了36.79%,图像噪声降低了21.96%,CNR及SNR分别提高了45.39%、47.89%,差异均有统计学意义(P<0.05);两种增强扫描图像的主观评分比较,差异无统计学意义(P>0.05)。结论:低千伏(80 kVp)肝脏CT增强扫描结合SAFIRE-3技术能得到较好的图像质量,同时可以降低有效辐射剂量。Objective: To explore the applied value of sinogram affirmed iterative reconstruction (SAFIRE) technique on the low kVp CT enhancement scan in liver. Methods: 54 patients with clini- cally suspected liver disease were divided into group A and group B, and underwent upper abdominal CT scan. In group A the voltage was 120 kVp and the image was reconstructed by filtered back projec- tion (FBP) technique while in group B the voltage was 80 kVp and the image was reconstructed by SAFIRE technology with intensity of level 3. The CT scan effective dose (ED) was compared between the two groups. The objective evaluation indexes (liver CT value, image noise, contrast noise ratio (CNR) and signal noise ratio (SNR)) and subjective evaluation indexes( image noise, vascular dis- play, constructed defect and image quality)to evaluate of the image quality of the two CT enhanced scanning method. Results: The effective dose of group B decreased by 36.79% compared with group A, and the difference was statistically significant (P 〈 O. 05 ) ; Compared with group A, image noise of group B was reduced by 21.96% , CNR and SNR were increased by 45.39% , 47.89% , respectively. The differences were statistically significant ( P 〈 0.05 ). There was no statistically significant differ- ence in subjective evaluation score between the two groups ( P 〉 0. 05 ). Conclusion: Low kV (80kVp) liver CT enhanced scan combined with SAFIRE-3 technique can produce better image quali- ty, and effectively reduce radiation dose.
