不同多普勒超声检查途径(经腹部、阴道)对妇科急腹症的诊断价值比较

摘　　要：目的观察比较不同彩色多普勒超声检查途径(经腹部、阴道)在妇科急腹症诊断中的应用价值。方法选取医院收治的136例经彩色多普勒超声检查的妇科急腹症患者资料与病理诊断结果进行比较,分析不同途径彩色多普勒超声检查(经腹部、阴道)对妇科急腹症的诊断价值。结果彩色多普勒超声经腹部检查确诊者98例,与病理诊断结果符合率为72.06%;彩色多普勒超声经阴道检查确诊者124例,与病理诊断结果符合率高达91.18%,2种方法比较差异有统计学意义(P<0.05)。经腹超声检查在子宫肌瘤变性及卵巢肿瘤蒂扭转诊断中的误诊率较高,经阴道超声在子宫肌瘤变性诊断中的误诊率较高。结论在妇科急腹症的临床诊断过程中,经阴道超声检查其诊断准确率高于经腹超声检查,且操作方法简便、安全,值得在临床上普及应用。Objective To observe and compare the value of the application of the color Doppler uhrasonography （via the abdomen,vagina） used in treatment with gynecological acute abdomen. Methods A total of 136 patients with gynecologi- cal acute abdomen who undeewent color Doppler ultrasonography were selected and compared with the results of pathological diagnosis. And the clinical diagnosis of colour Doppler ultrasonography （via the abdomen ,vagina） used for the treatment of gy- necological acute abdomen were analysed. Results Color Doppler ultrasonography in the abdominal examination confirmed 98 cases,with 72.06% of which that corresponds to the results of pathological diagnosis; Color Doppler ultrasonography confirmed by vaginal examination in 124 cases, with 91.18% of which that corresponds to the resuhs of pathological diagnosis;and signif- icant difference was statistically significant（P 〈 0.05 ）. The misdiagnosis rate of Transabdominal ultrasonography in the exami- nation of uterine fibroid degeneration and ovarian tumor torsion diagonosis is relatively high, and the middiagonisis rate of using transvaginal ultrasonography in the diagnosis of uterine fibroid degeneration is relatively the high. Conclusion In the course of clinical diagnosis of gynecological acute abdomen, the diagnostic accuracy of transvaginal uhrasonography is higher than that of transabdominal ultrasound,and the operation method is simple and safe,and it is worthy of clinical application.
