摘 要:目的探讨红细胞(RBCs)储存损伤对大肠杆菌感染的免疫力低下大鼠输血后死亡率的影响。方法将69只SPF级SD大鼠,根据是否使用免疫抑制剂(环磷酰胺和地塞米松)和输注RBCs的储存时间不同,随机分成3个实验组:免疫抑制+9 d RBCs组、免疫抑制+1 d RBCs组和免疫抑制+生理盐水(NS)组(18只/组);1个正常对照组:单纯NS组,含15只大鼠。每只大鼠经股静脉放出全身20%血液后,分别输注与失血量相同的储存9或1 d的浓缩RBCs及等量NS,随后植入大肠杆菌溶液0.5 mL,并连续观察7 d。术后24 h,取尾静脉血浆用亚铁嗪微板法测定血清铁,并以LB培养基血细菌培养15 h,用紫外分光光度仪测定OD600;术后24 h处死各免疫抑制组大鼠3只/组,取组织做病理检查。结果 3个免疫抑制组和对照组分别输注9、1 d RBCs和NS后的大鼠死亡率分别为10/15,11/15和9/15(P>0.05),死亡大鼠的平均生存时间(h)分别为55.6±21.3,38±16.6和28.3±15.0(P<0.05);24 h血清铁离子浓度(μmol/L)分别为34.1±9.8,28±10.2,26.8±9.0和22.4±5.9(P>0.05);血细菌培养的结果(OD_(600))分别为0.510±0.340,0.790±0.281,0.427±0.107和0.349±0.069(P<0.05);24 h回肠组织细菌计数(个/1 000倍视野)分别为44±9,101±7和27±8(P<0.05)。结论输注不同储存时间的RBCs对于大肠杆菌感染的免疫力低下大鼠的死亡率没有明显影响;但是相比储存时间长的RBCs,新鲜RBCs输注后明显促进了免疫力低下大鼠体内大肠杆菌的增值并加速了感染大鼠的死亡。Objective To investigate the influence of storage lesion of red blood cells (RBCs) on mortality of SpragueDawley rats infected by Escherichia Coli (E.coli).Methods 69 SPF-SD rats,according to use of cyclophosphamide and dexamethasone or transfusion with RBCs stored 9 days or less than 1 day or with normal saline (NS),were randomly divided into four groups:normal + NS;immunosuppression + 9d RBCs;immunosuppression + ld RBCs:immunosuppression +NS,and control group.Each of three immunosuppressive groups contained 18 rats,while the control group had 15 only.The rats,subjected to bloodletting 20% of total blood volume,were transfused with RBCs stored nine days,or less than one day or with NS,separately,and then were injected with E.coli (0.5 mL) through femoral vein.When transfused 24 h later,the blood of rats were collected from tail vein to determine serum iron and blood bacteria culture,and three rats of every immunosuppressive group were killed to make pathological specimen.Results For the immunosuppressive groups transfused 9d RBCs,ld RBCs,NS and control group,the mortality were 10/15,11/15 and 9/15,respectively (P〉0.05).The mean survival time (h) were 55.6±21.3,38± 16.6 and 28.3± 15.0,respectively (P〈0.05).The serum iron ion concentration (μmol/L)were 34.1±9.8,28± 10.2,26.8±9.0 and 22.4±5.9,respectively (P〉0.05).Meanwhile,the values (OD =600 nm) of blood culture were 0.510±0.340,0.790±0.281,0.427±0.107 and 0.349±0.069,respectively (P〈0.05).Conclusion No statistical difference in mortality of immunosuppressive rats infected by E.coli were found,when they were transfused with RBCs of stored different periods,separately.Nevertheless,infusion of RBCs with shorter storage period,compared with RBCs stored for longer time,promotes the proliferation of E.coli in immunosuppressive rats,and accelerates the death process of rats.
