摘 要:目的研究献血者戊肝病毒感染情况。方法对厦门地区部分献血者标本进行戊肝病毒抗原(HEV-Ag)、抗体(HEV-IgG和HEV-IgM)检测,HEV-Ag阳性和HEV-IgM阳性者进行HEV核酸检测,核酸检测阳性标本进行序列测定和基因型分析。结果总共6 263人份献血者标本中,检出1 447例HEV-IgG阳性(1 447/6 263,23.10%)、42例HEV-IgM阳性(42/6 263,0.67%)、12例HEV-Ag阳性(12/6 263,0.19%);12例HEV-Ag阳性标本中检出8例HEV核酸阳性(8/6 263,0.13%),42例HEV-IgM阳性标本中未检出HEV核酸阳性。8例HEV核酸阳性有4例测序成功,分别为2例Ⅰ型、2例Ⅳ型。随访显示大部分阳性献血者HEV病毒血症可较长时间持续存在。结论厦门地区献血者戊肝病毒现症感染率较高,在今后的输血保障工作中应引起重视。HEV-IgM检测对献血者HEV现症感染的检出率较低,而HEV-Ag的检出率较高且同HEV核酸检测具有较高的一致性,有望在血液筛查中得以推广。Objective To study the prevalence of hepatitis E virus infection in blood donors.Methods Blood samples from donors were tested for HEV-Ag,HEV-IgG and HEV-IgM.HEV-Ag or HEV-IgM positive samples were tested with HEV RNA.Phylogenetic analysis of HEV sequences obtained from HEV RNA positive samples were performed by direct sequencing.Results 1 447 (23.10%),42 (0.67%) and 12 (0.19%) out of 6 263 samples were tested positive for HEV-IgG positive,HEV-IgM,and HEV-Ag,respectively.HEV RNA was detected in 8/12 (66.7%) of the HEV-Ag positive samples whereas 0 was detected in HEV-IgM positive samples.4 of 8 HEV RNA positive samples were successfully sequenced,of which 2 were genotype Ⅰ and 2 were genotype Ⅳ.Results from follow-up indicated chronic viremia in HEV infected donors.Conclusion The study indicated a high prevalence of current HEV infection in Xiamen blood donors and attention should be paid in future work regarding blood safety.HEV-Ag test has a better coincidence rate with HEV RNA than HEV-IgM test,which has a better prospect of application in blood screening.
