5M1E分析法对重症监护病房老年患者发生肺部感染的防控效果

摘　　要：目的应用5M1E分析法对重症监护病房(ICU)住院的老年患者发生肺部感染的预防控制效果进行研究。方法以该院ICU科室收治的116例老年患者为研究对象,将采用5M1E分析法患者作为研究组,未采用5M1E分析法患者为对照组。采取5M1E分析法进行临床服务质量的管理,包括人员、机器、材料、方法、测量、环境6个方面。对患者发生肺部感染的情况、引起肺部感染的原因、治疗方法、治疗效果等指标进行统计分析。对发生肺部感染的比例进行比较。结果研究组老年患者发生肺部感染的共计32例,发生率为38.55%;对照组患者发生肺部感染共计53例,发生率为63.86%,两组比较差异有统计学意义(P<0.05)。研究组32例肺部感染患者中出现气促、咳嗽、咳痰、白细胞增多、胸片阳性、发热、胸闷、听诊湿啰音症状的概率较高,构成比超过90%。呼吸系统疾病、有气管插管、使用抗菌药物、冬春季是老年患者发生肺部感染的独立危险因素。结论5M1E分析法对预防老年患者发生肺部感染效果显著,但对于高危因素的防控应当更加重视。Objective To study the effect of 5M1E analysis method in prevention of pulmonary infection in elderly patients in ICU.Methods A total of 166 elderly patients were selected in ICU as subjects.Patients who received 5M1E analysis method were treated as the research group,and patients who didn′t receive 5M1E analysis method were treated as the control group.By 5M1E method,the person,machine,material,method,measure and environment were managed.And the pulmonary infection,causes,treatment and effects were analyzed by statistical software and compared between the two groups.Results A total of 32 cases（38.55%） occurred pulmonary infection in research group,and 53 cases （63.86%） occurred pulmonary infection in control group,there was significant difference between the two groups （P〈0.05）.It was higher than 90% that the rate of symptoms occurred,including short breath,cough,expectoration,leukocytosis,fever,chest tightness and rales auscultation.Respiratory disease,tracheal intubation,antibiotics,winter and spring were the independent risk factors of elderly patients in ICU with pulmonary infection.Conclusion 5M1E analysis method is effective in prevention of pulmonary infection in elderly patients in ICU,but prevention and control of high risk factors should be paid more attention.
