局部注射自体富血小板血浆联合运动疗法和局部注射曲安奈德治疗跟腱病的临床疗效对比

摘　　要：目的观察与评估自体富血小板血浆联合运动疗法与曲安奈德在治疗跟腱病中的临床疗效。方法选取2015年1月至2016年1月成都体育学院附属体育医院收治的共36例患者随机分为治疗组和对照组,对照组予以曲安奈德局部注射(40mg/次,3周/次)治疗,治疗组采用自体富血小板血浆(5m L/次,3周/次)配合小腿离心负荷运动训练干预(15次/组,3组/d,3d/周)。于治疗前及治疗后3周及6周后,分别采用美国后足评分系统(AOFAS)及视觉模拟评分系统(VAS)评估2组患者跟腱疼痛及足踝功能改善情况,并比较两组的总体疗效。结果治疗前两组组间各项评分比较差异无统计学意义(P>0.05)。治疗后1周、6周,两组组内评分均优于治疗前,差异有统计学意义(P<0.05),两组组间治疗后1周组间评分差异无统计学意义(P>0.05),治疗后6周评分比较差异有统计学意义(P<0.05)。结论自体富血小板血浆联合运动疗法和曲安奈德治疗跟腱病均有一定疗效,自体富血小板血浆联合运动疗法治疗跟腱病临床疗效优于曲安奈德。Objective To observe and investigate clinical therapeutic effectiveness of autologous platelet-rich plasma combined with kinesiotherapy and triameinolone acetonide on tendon disease. Methods 36 patients admitted to Sport Hospital At- tached to CDSU from January 2015 to January 2016 were randomly divided into treatment group and control group. The control group was given local injection of triamcinolone aeetonide （ once every three weeks and 40mg per time）while the treatment group re- ceived autologous platelet-rich plasma（ once every three weeks and 5ml per time）combined with leg eccentric training intervention （ 15 times per group,3 groups per day and 3 days per week）. Pain in the Achilles tendon and improvement of ankle in both two groups were investigated by applying American Orthopedic Foot and Ankle Society（AOFAS） Ankle Hindfoot Scale and Visual Ana- logue Scale（VAS） and total effects of them were compared before and two and three weeks after therapy. Results There were no statistics differences among all scores of two groups before therapy（ P 〉 O. 05 ）. One and six weeks after therapy ,the scores in each group were all greater than before（ P 〈 0. 05 ）. And there was no statistics difference in scores between two groups one week after therapy （ P 〉 0. 05 ） and the scores were statistically different between them six weeks after treatment. Conclusion Both autologous platelet-rich plasmas combined with kinesiotherapy and triamcinolone acetonide has effects on tendon disease and the frontier has better efficacy.
