术中超声指引下的系统性肝段切除治疗肝内胆管结石病的效果

摘　　要：目的探讨术中超声引导下系统性肝段切除术治疗肝内胆管结石病的临床疗效。方法选取2014年4月~2016年9月我院收治的系统性肝段切除术患者89例为研究对象,以采用的术式分组:对照组44例行胆管切开取石术,观察组45例行超声引导下系统性肝段切除术,比较两组治疗效果。结果 (1)观察组术中出血量[(126.73±32.87)ml]、手术时间[(82.25±13.17)min]、住院时间[(9.72±1.63)d]均显著少于对照组[(289.95±21.39)ml、(138.63±19.25)min、(13.76±2.26)d],差异有统计学意义(P<0.05);(2)观察组治疗总有效率(93.33%)显著高于对照组(72.73%)(P<0.05);(3)观察组并发症发生率(8.88%)显著低于对照组(29.55%)(P<0.05)。结论在超声引导下对肝内胆管结石病患者行系统性肝段切除术治疗,可提高结石清除率,缩短患者恢复时间,且安全性好,值得推广。Objective To investigate the clinical effect of intraoperative uhrasonography in the treatment of intrahepatic bile duct stones by systematic hepatic segment resection.Methods 48 patients with systemic hepatic resection were enrolled in our hospital from April 2014 to September 2016,The patients were divided into two groups： the control group （n=44） were treated with choledocholithotomy,the observation group （n=45） were underwent ultrasound-guided systemic liver resection,the effect in two groups were compared.Results ~ The intraoperative blood loss [（126.73＋-32.87） ml], the operation time [（82.25＋-3.17） mini and the hospitalization time [（9.72＋-1.63） d] were significantly lower than those in the control group [（289.95＋-21.39） ml, （138.63＋19.25） min,（13.76＋2.26） d] （P〈0.OS）.（~）The total effective rate was 93.33% in the observation group and higher than 72.73% in the control group.（~）The incidence of complication in the observation group （8.88%） was significantly lower than that of the control group （29.55%） （P〈0.05）.Conclusion Ultrasound-guided treat- ment of intrahepatic bile duet stones in patients with systemic liver resection, can improve the removal rate of stones, shorten the recovery time of patients, and the safety is good, worthy of promotion.
