腹腔镜胆囊切除术在胆囊结石合并肝硬化患者中的安全性分析

摘　　要：目的分析胆囊结石合并肝硬化患者应用腹腔镜胆囊切除术的临床意义。方法从我院2015年11月至2016年9月收治的胆囊结石合并肝硬化患者中抽取72例作为研究对象,根据不同术式均分为对照组和观察组,各36例。对照组采取开腹传统手术治疗,观察组予以腹腔镜胆囊切除术治疗,观察两组手术情况、生化指标及不良事件发生情况。结果观察组手术时间、失血量为、住院时间均优于对照组(P<0.05);两组治疗1周后总胆红素、凝血酶原时间及谷丙转氨酶明显优于治疗前,且观察组均优于对照组(P<0.05);观察组不良事件总发生率为5.56%,低于对照组的22.22%(P<0.05)。结论在胆囊结石合并肝硬化患者的治疗中,腹腔镜胆囊切除术可快速控制病情,且安全性较高。Objective To analyze the clinical significance of laparoscopic cholecystectomy on cholelithiasis complicated with liver cirrhosis. Methods Seventy-two cases of patients with gallstone cirrhosis in our hospital from November 2015 to September 2016 were selected as research objects, which were divided into control group and observation group, according to the different type of operation. The control group was treated with traditional open surgery, while the observation group received laparoscopic cholecystectomy excision, the operation indexes, biochemical indexes and adverse events were observed between the two groups. Results The duration of operation, blood loss and hospitalization time of the observation group were better than those of the control group （P〈0.05）. A week after the treatment, the total bilirubin, prothrombin time and alanine aminotransferase of the two groups were better than those before treatment, and those of the observation group were better than the control group （P〈0.05）. The total incidence rate of adverse events of the observation group was 5.56%, which was lower than 22.22% of the control group （P〈0.05）.Conclusion In the treatment of patients with cholelithiasis complicated with liver cirrhosis, laparoscopic cholecystectomy can quickly control the disease, which has high safety.
