ESAT-6/CFP-10/Rv3425/Rv3248c辅助诊断结核病的初探

摘　　要：用酶联免疫斑点试验(enzyme-linked immunospot assay,ELISPOT)评估新型复合抗原ESAT-6/CFP-10/Rv3425/Rv3248c在活动性肺结核病诊断中的临床价值。选取初治肺结核患者血常规标本50例和健康体检者血常规标本40例,分别作为结核组和健康对照组,分离PBMC,应用新型复合ESAT-6/CFP-10/Rv3425/Rv3248c与ESAT-6/CFP-10融合蛋白作为抗原对受试者标本进行ELISPOT实验,检测两者斑点形成的细胞数量并进行比较。结果显示,复合抗原ESAT-6/CFP-10/Rv3425/Rv3248c与ESAT-6/CFP-10融合蛋白检测结核分枝杆菌感染的阳性率依次为86%(43/50)、82%(41/50),特异度依次为82.5%(33/40)、85%(34/40)。研究表明,应用新型复合蛋白ESAT-6/CFP-10/Rv3425/Rv3248c作为抗原建立的ELISPOT法辅助诊断结核病具有较高的潜在应用价值。To evaluate the application value of a new compound antigen ESAT-6/CFP-10/Rv3425/Rv3248c in the detection of active pulmonary tuberculosis, a total of 50 patients with initial pulmonary tuberculosis and 40 healthy persons were enrolled as tuberculosis group and control group respectively. The peripheral blood mononuclear cells of all participants were co-cultured with the compound antigens ESAT-6/CFP-10/Rv3425/Rv3248c and ESAT-6/CFP-10 respectively. SFCs were enumerated by ELISPOT. The results showed that the detection sensitivity of the new compound antigen ESAT-6/CFP-10/Rv3425/Rv3248c and ESAT-6/CFP-10 antigen by ELISPOT was 86 % （43/50）and 82 % （41/50） respectively, and the specificity was 82.5 % （33/ 40）and 85 % （34/40） respectively. So, ELISPOT with the new compound antigen ESAT-6/CFP-10/Rv3425/Rv3248c shows a potential value for auxiliary diagnosis of pulmonary tuberculosis.
