摘  要:目的:探讨尼古丁对脓毒症大鼠血糖和血糖相关类激素的影响。方法:雄性Wistar大鼠随机分3组,每组10只:假手术组、脓毒症组、尼古丁组。盲肠结扎穿孔法建立脓毒症模型,尼古丁组在造模型成功后腹腔注射尼古丁(400μg/kg)。术前及术后2h测血糖,ELISA测血清胰岛素、胰高血糖素和皮质醇浓度,计算胰岛素抵抗指数。结果:(1)术前3组大鼠血糖无统计学差异(P>0.05);脓毒症组术后2h血糖水平显著高于假手术组和尼古丁组(P<0.05)。(2)脓毒症组胰高血糖素、皮质醇和胰岛素浓度显著高于假手术组,并存在胰岛素抵抗。尼古丁组可抑制应激激素水平升高,改善胰岛素抵抗(均P<0.05)。结论:尼古丁可改善脓毒症大鼠早期高血糖和胰岛素抵抗,机制可能与其平衡应激激素有关。Objective: To investigate the effect of nicotine on blood glucose and counterregulatory hormones in septic rats. Methods: Male Wistar rats were randomly divided into three groups (n=10 each): sham group, CLP group, and nicotine group. Rats in nicotine group were treated with nicotine (400 μg/kg,i. p. ) after the operation of cecal ligation and puncture. The blood glucose was measured 2 h before and after the operation. Meanwhile, the serum concentrations of glucagon, cortisol and insulin were measured by ELISA. The insulin resistance index of 2 h after sham surgery and CLP were calculated. Results: There was no statistic difference before the operation on the level of blood glucose(P〉0.05). The level of blood sugar 2 h after CLP was significantly higher than sham group and nicotine group(P〈0.05 ,respectively). The level of serum glucagon, cortisol, insulin and insulin resistance index in group CLP were significantly higher than group sham (P〈0.05), which can be improved by nicotine (P〈0.05). Conclusion: The phenomenon of stress hyperglycemia or insulin resistance appears at early phase of sepsis, which can be im-proved by nicotine through the intraperitoneal injection. The mechanism may be associated with balancing stress hormones.
