摘  要:目的:定性分析人体肺部超极化^(129)Xe MRI(HP^(129)Xe MRI)肺通气成像可行性和安全性。方法:8例受试者(5例健康志愿者、1例慢性支气管炎、1例哮喘及1例吸烟志愿者)行低剂量CT扫描和及HP^(129)Xe MRI肺通气成像;并与低剂量CT图像对照,分析HP^(129)Xe MRI肺通气成像特征,分析检查前后心率、收缩压、舒张压及血氧饱和度等生理指标变化。结果:所有受试者检查前后心率、血氧饱和度及血压等生理指标未见明显变化,受试者除2例主诉头昏外,无其他不适(P>0.05)。8例受试者肺通气HP^(129)Xe相肺气体信号分布呈由前向后递增趋势,反映其重力顺应性梯度。5例健康志愿者双肺HP^(129)Xe分布均匀,1例慢性支气管炎患者CT显示左下肺片状的低密度区,HP^(129)Xe MRI不仅显示CT上已发现病变,同时还发现肺内更小通气缺损区。1例哮喘患者CT未见异常,而HP^(129)Xe MRI显示肺内多发通气缺损区。1例吸烟者CT显示双上肺胸膜下低密度区,HP^(129)Xe MRI能显示双肺胸膜下广泛通气缺损区。结论:HP^(129)Xe肺部MRI通气成像能安全有效评估肺通气缺陷,可视化显示CT未能显示的早期、多发、小灶性肺通气缺陷病变,亦能反映肺部重力顺应性梯度。Objective: To preliminarily explore the feasibility and safety in human lung ventilation imaging with hyperpolarized 129Xe MRI (HP 129Xe MRI). Methods: Eight subjects(5 healthy volunteers, one chronic bronchitis patient, an asthma patient, and a smoker) were underwent CT and HP 129Xe MRI examinations. The results were compared with low dose CT imaging characteristics. Physiological parameter changes in heart rate, blood pressure, and blood oxygen saturation before and after the HP 129 Xe MRI were analyzed. Results: Physiological parameters had no changes before and after the examinations of all subjects (P〉0.05). Except two subjects complained of dizziness, the other volunteers didn't report discomfort. The pulmonary gas of HP 129Xe MRI signal distribution of all subjects presented gradient-like increasing trend from the anterior to the posterior lung, which reflected the pulmonary compliance with gravity gradient. The HP 129Xe MRI signal distribution in 5 healthy volunteers was homogeneous. The patient with chronic bron-chitis showed low density area in the lower left lung on CT, but HP 129 Xe MRI can show smaller lung ventilation defects. CT of the asthma presented no abnormalities, and HP 129Xe MRI can find ventilation defect areas in both lungs. Low density areas at the pleural translucent area in the upper lung can be found in CT images of the smoker. In addition to displaying these lesions, HP 129Xe MRI can find more small ventilation defects. Conclusion: HP ~29Xe human pulmonary MR ventilation imaging is a safe and effective method on evaluating the lung ventilation defects. HP 129Xe MRI can sensitively show earlier and smaller ventilation defects than CT and maybe reflect the pulmonary compliance with gravity gradient.
