Speckle Tracking Echocardiography Assessment of Left Ventrical Contractile Function Changes in Pressure-Overloaded Heart Failure Mice

摘  要:目的:利用斑点追踪成像技术探讨压力负荷型心衰小鼠的心衰发展进程中左心室整体及局部心功能的变化情况,为预测心衰的发展提供新思路。方法:用小鼠重度胸主动脉缩窄模型模拟压力负荷型心衰发生发展情况,术后1-12周每周监测手术组和假手术组小鼠常规心脏超声和斑点追踪成像参数的变化情况。结果:手术组小鼠术后1周即出现射血分数、纵向应变、径向应变、环向应变等左心室整体心功能参数的降低以及心尖部、心中部、心底部局部应变的下降,随着时间的进展,术后第7周出现整体纵向应变和心尖部纵向应变的进一步下降,第8周出现整体和局部各段心功能的全面下降。结论:在心衰的发展过程中,纵向应变先于常规心脏超声参数和径向应变、环向应变发生失代偿性损伤,心尖部纵向应变先于心中部和心底部出现损伤。Objective: To explore the changes of global and regional left ventrieular function in pressure-overloaded heart failure(HF) mice by speckle-tracking echocardiography (STE), and provide possibility for predicting the development of HF. Methods: Adult mice were subjected to severe thoracic aortic banding (sTAB). After surgery, conventional echocardiography and STE were performed weekly in the sTAB group and sham group. Results: In the sTAB group, left ventricular(LV) contractile function parameters including ejection fraction (EF), longitudinal strain (LS), radial strain (RS) and circumferential strain(CS) were significantly decreased 1 week after surgery. At the end of the 7 th week after surgery, global LS and LS of apex region were further decreased, and at the 8 th week, LV contractile function parameters were all declined. Conclusion: In the development of pressure-overloaded HF, changes of LS were appeared earlier than conventional echocardiography, RS and CS. Changes of apex strain were appeared earlier than mid and base strain.
