壮肾灵外敷对急性心肌梗死再灌注损伤研究

摘　　要：目的:探讨壮肾灵外敷心俞对急性心肌梗死再灌注损伤的影响。方法:选择60例溶栓的急性ST段抬高型心肌梗死病例,治疗30例患者,采用尿激酶静脉溶栓联合壮肾灵外敷心俞,对照组30例患者,采用单纯溶栓。比较两组患者治疗前后的心肌酶、BNP、心脏彩超、心肌梗死面积等变化。结果:两组患者治疗前的LVEF比较,差异无统计学意义(P>0.05);治疗后的LVEF与治疗前比较,差异具有统计学意义(P<0.05);治疗组患者治疗后的LVEF明显高于对照组治疗后,经比较,差异具有统计学意义(P<0.05)。6个月的随访过程中,对照组心脏事件的发生率为30.0%,治疗组心脏事件的发生率为20.0%,明显低于对照组,经比较,差异具有统计学意义(P<0.05)。结论:壮肾灵外敷可以改善急性心肌梗死再灌注损伤患者的心功能,提供再灌注损伤的保护作用,尤其是可以降低心脏事件的发生率,值得临床推广应用。Objective To investigate the effect of Zhuang shenling external hsin shu reperfusion of acute myocardial infarc-tion. Method 60 cases of thrombolysis in acute ST segment elevation myocardial infarction cases, treatment of 30 patients, using urokinase intravenous thrombolysis combined Zhuang shenling topical hsin shu, control group, 30 cases of patients using pure thrombolysis. Comparison of two groups before and after treatment in patients with myocardial enzyme, BNP, heart colour to ex-ceed ,the changes of myocardial infarction and area. Results LVEF compared two groups of patients before treatment, there was no statistically significant difference （ P 〉 0. 05 ） ; LVEF compared with before treatment, after treatment, the difference was statisti-cally significanct （P 〈0. 05） ;Treatment group after treatment in patients with LVEF were significantly higher than those of the control group after treatment,by comparison, the difference was statistically significant （P 〈0. 05）. During 6 months of follow - up,the control group,the incidence of cardiac event was 30. 0 % , in treatment group, the incidence of cardiac event was 20. 0 % , significantly lower than the control group, by comparison, the difference was statistically significant （ P 〈 0. 05 ）.Conclusion Zhuang shenling external treatment can improve cardiac function in patients with acute myocardial infarction reper-fusion, provides the protective effects of reperfusion injury,especially to reduce the incidence of heart attacks,worthy of clinical popularization and application.
