摘　　要：目的探讨日间手术中心开展腹腔镜下子宫手术的可行性和安全性。方法 2015年10月至2016年12月在上海交通大学医学院附属仁济医院东院日间手术中心共完成腹腔镜下全子宫切除术和子宫肌瘤剔除术95例。患者年龄(45.44±8.83)岁。其中行全子宫切除术45例,行子宫肌瘤剔除术50例。患者在门诊完成术前准备,手术当日入院,术后48h内出院。结果所有患者手术过程顺利,术中出血少,无手术并发症,没有中转开腹,术后入住日间病房。术后无发热,排气、排便恢复正常。除2例转回普通妇科病房,其余均按期出院,无出院后再入院病例。患者住院天数大大减少,医疗费用显著下降。结论日间中心完成腹腔镜下子宫手术是完全可行、安全的,有较显著的社会效应和经济效益。Objective To investigate the feasibility and safety of laparoscopic uterine procedures in ambulatory surgery center.Methods A total of 95 patients received laparoscopic hysterectomy and myomectomy in the ambulatory surgery center in the eastern part of Renji Hospital, Shanghai Jiaotong University School of Medicine, from October 2015 to December 2016.The average age was 45.44±8.83 years old.Among these cases, 45 cases had hysterectomy, and 50 cases had myomectomy.All the patients completed preoperative preparation in outpatient department, and were admitted at the day of surgery.The patients left hospital within 48 hours after operation.Results All these procedures were performed successfully, with less bleeding ; there were no complications or conversion to open surgery.The patients were moved to the day surgery ward postoperatively.There were no fever.Urination and defecation.Except for 2 cases, all were regularly discharged, and there was no readmission after discharge.The hospitalization days were decreased significantly, and the medical spending was also reduced significantly.Conclusion It is feasible and safe to perform laparoseopic hysterectomy and myomectomy in ambulatory surgery center.The social and economic benefits are significant.
