摘 要:目的观察系统饮食指导对妊娠期糖尿病(gestational diabetes,GDM)患者依从性及母婴结局的影响.方法选取2015-01/2016-01在安吉县人民医院分娩的112例GDM患者作为研究对象,所有患者随机分为观察组和对照组.观察组接受系统饮食指导,对照组接受常规饮食指导.比较2组患者干预前后的体质量指数(body mass index,BMI)、血压、血脂代谢水平、血糖控制情况(空腹血糖、餐后2 h血糖及夜间血糖)、依从性评分及母婴不良反应的发生率.结果观察组患者依从性评分9.1±1.4,对照组6.2±1.8,差异具有统计学意义(t=5.254,P<0.05).与干预前相比,2组患者干预后的空腹血糖、餐后2 h血糖、夜间血糖、糖化血红蛋白、总胆固醇及低密度脂蛋白胆固醇显著降低(P<0.05),BMI上升(P<0.05),2组患者干预后上述指标的差异具有统计学意义(P<0.05).2组患者羊水过多及剖宫产发生率的差异具有统计学意义(P<0.05).观察组新生儿体质量3154 g±327 g,对照组3741 g±478 g,差异具有统计学意义(P<0.05).结论系统饮食指导对GDM患者的依从性及母婴结局具有明显的改善作用.AIM To study the effect of systematic diet guidance on compliance and maternal-infant outcomes in patients with gestational diabetes mellitus (GDM).METHODS From January 2015 to January 2016, 112 GDM patients were included and randomly divided into two groups: a study group (n=56) and a control group (n = 56). The study group received systematic diet guidance, and the control group received routine diet guidance. BMI, SBP, DBP, TG, TC, HDL-C, LDL-C, FPG, 2 h PG, nPG, compliance score, and maternal-infant outcomes were compared between the two groups.RESULTS The compliance score was significantly higher in the study group than in the control group (9.1 ± 1.4 vs 6.2 ± 1.8, t = 5.254, P 〈 0.05). After intervention, FPG, 2 hPG, nPG, HbAlc, TC and LDL-C decreased significantly (P 〈 0.05), and BMI increased significantly (P 〈 0.05) in both groups, and the improvements of these indexes were more significant in the study group than in the control group (P 〈 0.05). The birth weight of newborns was significantly lower in the study group than in the control group (3154 g ± 327 g vs 3741 g ± 478 g, t = 4.984, P 〈 0.05). The rates of polyhydramnios and cesarean section differed significantly between the two groups (P 〈 0.05).CONCLUSION Systematic diet guidance can help improve compliance and maternal-infant outcomes in patients with GDM.
